About this file

This document is a Minority Business Enterprise (MBE) Form issued by the City of Fort Myers, which establishes a 15% MBE goal per Resolution 38-125. The form is designed to help contractors document their good faith efforts in meeting this minority business participation target. It provides a structured approach for prime contractors and subcontractors to record their engagement with minority-owned businesses, including detailed steps for documenting communication, solicitation efforts, and potential subcontractor interactions.

The form requires contractors to provide comprehensive information about their outreach to MBE firms, including transmittal letters, contact methods, quotation reviews, and reasons for utilizing or not utilizing specific minority businesses. Key sections include documenting the total contract amount, specific dollar amounts for labor/services and materials, project commitment and completion dates, and a systematic approach to demonstrating meaningful efforts to include minority-owned businesses in the procurement process. The form serves as both a documentation tool and a mechanism for ensuring transparent and equitable contracting practices in municipal projects.
