About this file

This document is a Minority Business Enterprise (MBE) Form for the City of Fort Myers, Florida, specifically related to a brick paver maintenance and installation contract. The form is designed to document contractors' good faith efforts in meeting the city's MBE goal of 15%, requiring detailed documentation of outreach to minority-owned businesses. The form includes sections for prime contractor information, MBE contractor details, record of transmittal letters, telephone/in-person visit records, and explanations for incomplete quotations or non-utilized MBE firms.

The form is part of a procurement process that aims to ensure diverse business participation in the city's brick paver maintenance and new installation project. Contractors must comprehensively document their efforts to engage minority-owned businesses, including specific details about communication attempts, quotations received, and reasons for selection or non-selection of MBE subcontractors.
