About this file

This document is a Minority Business Enterprise (MBE) Form issued by the City of Fort Myers, Florida, for the Fleishman Park Improvements project. The form is designed to document good faith efforts in achieving the city's 15% MBE goal for contract opportunities. The form requires prime contractors or subcontractors to provide detailed information about their outreach to minority-owned businesses, including contact methods, transmittal letters, telephone or in-person visit records, and explanations for any incomplete quotations or non-utilized MBE firms.

The form serves as a compliance document for contractors to demonstrate their commitment to engaging minority-owned businesses in the bidding and contracting process. It includes six steps for documentation: prime contractor information, MBE contractor information, record of transmittal letters, telephone/in-person visit records, handling of incomplete quotations, and explanations for non-utilized MBE firms. The form is specifically linked to the Fleishman Park Improvements project, which involves demolishing and replacing roof structures over bleachers and dugouts, and requires contractors to show their efforts in meeting the city's minority business participation goals.
