About this file
The document is a Living Wage Certification Form from the Town of Manchester, Massachusetts, specifically related to the Manchester Living Wage Ordinance (Chapter 212, Sections 212-1 through 212-11). The form requires bidders to indicate whether they are a "Covered Employer" subject to the ordinance's requirements or qualify for an exemption, with a clear warning that failure to complete and return the form may result in bid rejection.
Exemptions from the living wage requirement include bidders who: employ fewer than 25 eligible employees, have an annual contract value less than $25,000, or are charitable foundations/trusts/nonprofit agencies that are exempt from federal income taxation under Section 501 of the Internal Revenue Code. To be compliant, covered employers must pay the required living wage to eligible employees throughout the contract term. The form requires the bidder's authorized representative to sign and certify the accuracy of their classification, with spaces provided for the company name, bid/RFP number, and other identifying information.
