About this file

This document is a certification form requiring the prime contractor to attest to compliance with limitations on subcontracting rules under the relevant Federal Acquisition Regulation and Veterans Affairs Acquisition Regulation clauses. The prime contractor must certify that either no subcontractors will be used, or that similarly situated subcontractors will receive at least 50% of the total contract value. If subcontractors are used, the contractor must provide the business names, SAM UEI numbers if available, descriptions of work, and percentage of total contract cost for each subcontractor. The total percentages performed by the prime, similarly situated subcontractors, and other than similarly situated subcontractors must be provided. Failure to comply with subcontracting limitations can result in penalties up to $500,000 or the amount spent in excess of permitted levels on subcontractors. The related federal contract opportunity is for linen transportation services for the St. Albans VA Textile Care Facility, issued under solicitation number 36C24224Q0061 by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2.
