About this file

This is an LGBTQ Applicant Opportunity Data Form used by the City of Philadelphia to collect diversity and inclusion information from vendors and contractors seeking to do business with the municipality. The form is designed to identify companies beneficially owned and controlled by individuals who self-identify as Lesbian, Gay, Bisexual, Transgender, or Queer (LGBTQ) and to track their participation in City procurement opportunities. Applicants are required to disclose whether their business is certified as an LGBTBE by the National LGBT Chamber of Commerce (NGLCC) through the Independence Business Alliance (IBA), their local Philadelphia affiliate, and whether they hold certifications from other third-party certifying agencies such as ESMDC, PAUCP, Disability:IN, or WBENC. Additionally, applicants must identify any subcontractors that are certified as LGBTBE by NGLCC/IBA.

Submission of the form is entirely voluntary, and applicants consent to the City's discretionary use of the collected data for other projects and potential public disclosure. The data serves to establish baseline information on LGBTQ-owned businesses currently engaged with the City and to facilitate their inclusion in the City's vendor database as part of Philadelphia's commitment to diversity, equity, and inclusion in all procurement activities. The form requires applicants to provide their business name, bid or opportunity number, certification status, and details regarding any certified subcontractors.
