About this file

This is an LGBTQ Applicant Opportunity Data Sheet issued by the City of Philadelphia for a micro purchase opportunity (B2627080) regarding an Elevator Safety Rescue Block. The form serves as a voluntary data collection instrument to identify companies beneficially owned and controlled by Lesbian, Gay, Bisexual, Transgender, and Queer (LGBTQ) persons seeking to conduct business with the City. The micro purchase will be awarded as a whole or by line item, whichever serves the city's best interest, to the lowest responsible bidder. Sellers must quote all items to be eligible for award and should not deliver or provide services until officially notified by the Department.

The data sheet requests information regarding LGBTQ business certification status, including whether the applicant's business is certified as an LGBTBE by the National LGBT Chamber of Commerce (NGLCC) through the Philadelphia affiliate Independence Business Alliance (IBA), or by other third-party certifying agencies such as ESMDC, PAUCP, Disability:IN, or WBENC. Applicants are also asked to identify any LGBTQ-certified subcontractors. The City of Philadelphia intends to use the collected data to identify LGBTQ businesses currently engaged with the City, enhance inclusion efforts in the vendor database, and may make the data publicly available at its discretion. Submission of this form is entirely voluntary and does not obligate participation in the contracting opportunity.
