About this file
This is a Letter of Supply Template that serves as Attachment 3 to RFP 75D30126Q79181 for the CDC's Clinical Trial Management System (CTMS) procurement. The template is designed to be completed by an Original Software Manufacturer (OSM) on their company letterhead and submitted to the Contracting Officer at the Centers for Disease Control and Prevention to verify the OSM's authorization of a small business prime contractor to market, resell, configure, and support the cloud-based CTMS platform.
The template requires the OSM to formally commit to four key obligations throughout the three-year period of performance, including all option periods: (1) guaranteed uninterrupted source of supply of the CTMS Software-as-a-Service application to meet all requirements under CLINs 0001, 1001, and 2001; (2) provision of back-end technical support, system maintenance, and software updates to maintain the required 99.5% system availability per the contract's Quality Assurance Surveillance Plan (QASP); (3) continuous maintenance of active FedRAMP authorization and full cooperation with Third-Party Assessment Organizations for monthly continuous monitoring data deliveries to support security and compliance requirements; and (4) transition assistance upon contract expiration or closeout, including secure data extraction and sanitization in accordance with NIST SP 800-88 guidelines. This template document does not address pricing, response deadlines, or award dates, as it is a standardized form for offerors to use during the solicitation process.
