About this file
This is a Legal Disclosure Instructions document issued by the City of Harrisburg Office of Purchasing for a Speed Cushions Installation Project in Pennsylvania. The document outlines mandatory legal disclosure requirements that firms responding to the request for proposals must submit as a signed self-certification letter. Respondents must disclose fines and disbarments levied by governmental agencies such as the EEOC or Department of Labor related to the proposed work, outstanding legal disputes with municipalities including the City of Harrisburg, judgments and ongoing litigation relative to the services proposed covering the last five calendar years, and any contracts for the services proposed that were terminated early over the past five calendar years with reasons for termination. The project itself involves constructing asphalt speed cushions in existing pavement at specified locations in accordance with Special Provisions, PennDOT Publication 408, the latest edition of the MUTCD, and approved Speed Cushion Details.
All submitted information must be reviewed and verified for accuracy and completeness before the self-certification letter is signed and submitted. Interested prime and subcontractors are required to identify their intent to bid by logging into the project profile on PennBid, which serves as the sole source of accurate information regarding the bid. The document directs all respondents to review supporting documentation, project details, and a questions and answers guide prior to submitting inquiries or attending a site visit, with a specific note that bids containing omissions resulting from industry assumptions will be deemed non-responsive.
