Instructions **NOTE: This tab contains instructions for completing Table ES-1, Offeror Summary** Instructions for Table ES-1, Offeror Summary.
Column A List the Prime, all subcontractors (principal subcontractors, critical subcontractors, and all other subcontractors), JV team members, and others who will be involved with performance or management of the requirements of this solicitation. Add rows as necessary. (Should account for 100% of Prime's capability gaps.)
Column B List all Places of Performance, being careful to include accurate physical locations.
Note: Place of Performance for this solicitation pertains to the physical location of the Prime, principal subcontractor, critical subcontractor, and JV team members that are deploying personnel or responsible for the work; not the Government site where the work is being performed. If work is being performed at a contractor site, then that would also pertain to Place of Performance.
Column C Provide the CAGE Codes/DUNS numbers associated with the entity that is proposing to perform the work for this solicitation.
Note: Past performance of any entity with multiple CAGE codes/DUNS will be considered for the same physical location if they have distinct roles and responsibilities. If multiple CAGE codes/DUNS for the same physical location will be involved with performance or management of the requirements of this solicitation, ensure they are identified on separate lines in Table ES-1, Offeror Summary.
Column D Brief Work Description and/or Program Role and Responsibility for each CAGE Code/DUNs identified.
Note: Provide a definition of the legal relationship between the entities if it is other than a Prime/subcontractor relationship.
