About this file
This is a Scope of Work document for Johnson Controls related to fire systems inspections under Illinois DMH AMHC contract requisition #27-444DHS-MENTH-R-325387. The scope encompasses two primary service categories: fire alarm system inspections and damper inspections. Fire alarm testing involves diagnostic examinations and inspections of accessible peripheral devices currently connected to facility fire alarm systems, with advance scheduling required. Documentation must include device locations with system addresses or zone designations, test results with voltage readings, and notation of any discrepancies. Damper inspection services follow NFPA 105 2010 and NFPA 80 2010 standards and include locating and actuating dampers, removing and resetting fusible links on fire dampers, clearing debris from damper operations, coordinating with customer representatives and local fire departments for system testing, manually activating electric and pneumatic actuators, and marking damper locations on Life Safety Drawings with identification stickers. The scope is limited to containment dampers (fire and smoke dampers) only.
The contractor is responsible for installing appropriately sized access doors to dampers where existing access is unavailable, with access door costs charged against a repair contingency fund. Battery replacement for wireless devices is excluded from the agreement scope and represents an additional cost. Client inspection reports are provided online at no charge; however, printed and mailed reports incur a cost of $5.00 per page. The contractor must comply with Authority Having Jurisdiction requirements and any internal organizational standards that may exceed state requirements. For evacuation and purge dampers requiring inspection, the customer must provide personnel to activate smoke control systems at the panel.

STANDARD DAMPER INSPECTION SERVICES
Based on Customer’s Life Safety Drawings and other information provided to LSS by Customer, provide the following inspection services with respect to Customer’s dampers (collectively, “Inspection Services” reference NFPA 105 2010and NFPA 80 2010):
1. Use its commercially reasonable efforts to locate and, once located, operate(i.e., actuate) dampers in the Customer’s facility or otherwise identify inoperable dampers.
2. Remove and reset fusible links on fire dampers, to verify each fire damper fully closes.
3. Clear each damper of any non-adhered to debris that would impede such damper’s normal operation.
4. Coordinate with Customer’s representative and local fire department to place Customer’s fire alarm system in “test mode” if necessary to conduct inspection of the smoke and/or combination dampers.
5. Manually activate actuators on electric and pneumatic smoke and/or combination dampers to verify proper operation of such damper.
6. Identify on Customer’s Life Safety Drawings the location of each damper, along with placement by LSS of identification stickers on ceilings or other location to assist in locating such dampers following inspection.
7. This Proposal is only for containment dampers (e.g. fire and smoke dampers). If evacuation and/or purge dampers are required for inspection, Customer must provide personnel to activate the smoke control/alarm at the panel.
8. If there is no existing access to a Damper, then proper size access doors, as reasonably determined by LSS, will be installed to facilitate inspection. Access door cost will be charged against the repair contingency fund.
9. Client reports will be made available online at no additional charge to Customer.
10. Standard Service Agreements do not include printed reports. Reporting will only be available online and can be printed by customer at customer's expense. Should customer prefer LSS to print and mail the reports, there will be a cost of $5.00 per page.
