About this file
This is a Past Performance Questionnaire (PPQ) template used by the U.S. Government Publishing Office's Acquisition Services division to collect contractor performance evaluations as part of source selection processes. Offerors are responsible for including completed CPAR (Contractor Performance Assessment Report) and/or PPQ forms as part of their quotes; PPQs must not be sent directly to the contracting office. Once completed, the information is classified as source selection sensitive information under 41 U.S.C. 423 and must be protected accordingly.
The questionnaire is divided into two sections: the first collects contract details to be completed by the contractor, including contractor name and address, project name, contract number and date, period of performance, completion date, contract value, contract type, and a description of key project characteristics. The second section is completed by the evaluating organization representative and captures the evaluator's name, position, contact information, and months of monitoring. The evaluation component uses a six-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Observed) to assess nine performance areas: quality of products and services, overall performance, schedule adherence, technical requirements fulfillment, cost control, subcontracting compliance, customer satisfaction in routine matters, customer satisfaction during unexpected difficulties, and repeat business likelihood. The form also includes sections for listing contractor weak points, strong points, other completed projects, and average award fee percentage if applicable.
