About this file
This is a Performance Questionnaire template used by the Federal Aviation Administration (FAA) to evaluate contractor performance in support of source selection decisions.
The questionnaire is designed for the Air Cooled Chiller Replacement project at FAR ATCT and requires completion and submission to Erin Crabtree, FAA Contracting Officer, at Erin.M.Crabtree@faa.gov. The offeror section requests basic contract information including contractor name and address, contract number, contract initiation and completion dates, total contract value, and contract type (Fixed Price, GMP, FPI, etc.), along with a detailed project description. The evaluating organization representative section requires identification of the evaluator's name, position (Project Manager, COR, or other), organization, phone number, and the number of months the evaluator monitored contractor performance. The evaluation criteria consist of seven assessment categories—Quality of Products and Services, Performance, Technical Requirements, Schedule, Cost Control, Customer Satisfaction, and Overall Assessment—each to be rated using a six-point scale: Exceptional (EX), Very Good (VG), Satisfactory (S), Marginal (MG), Unsatisfactory (US), or Not Observed (N/O). The form allows for additional comments on a supplementary page if needed.
