About this file

This document is a Request for Quote (RFQ) issued by the Illinois Department of Veterans Affairs for the procurement of influenza vaccine for the 2021 season. The RFQ specifies two types of influenza vaccine, Fluad and Flucell, to be provided in packs of 10 doses each. The vendor must provide pricing per pack and comply with prevailing wage requirements. The RFQ includes instructions for completing the quote form, warranty requirements, and delivery timelines. Quotes must be valid for 180 days, and the agency reserves the right to reject any or all offers.

The pricing terms indicate that all quoted prices must include shipping and handling charges, and no price increases will be accepted. The agency also guarantees the lowest price in the event of a price decrease. The winning vendor will be selected based on the lowest total bid for all products. There is no mention of any set-asides, incumbents, or funding sources in the document.
