About this file
This is a Pricing Submittal Form (Attachment D) for an Invitation to Bid issued by the Oregon Department of Fish and Wildlife (ODFW) for the Summer Lake Wildlife Area 6-bay Garage Concrete Floor project (File #25-001). The project encompasses construction of interior and exterior concrete slabs, with the base bid including mobilization/demobilization/bonding, an interior concrete slab of approximately 1,342 square feet, and an exterior concrete slab of approximately 460 square feet. Two bid alternates are offered: Bid Alternate #1 involves replacement of an existing concrete apron across the front of the 6-bay garage building (approximately 120 square feet), and Bid Alternate #2 involves adding a 9'6" x 23' extension to the exterior concrete slab line item. Bidders are required to enter unit prices and total prices for all listed items, with the form allowing for adjustment of quantities and deletion of items at the discretion of ODFW to meet budgetary constraints. Award will be based on the greatest monetary value achievable within available budget.
The pricing submittal requires bidders to provide costs for all base bid items and applicable alternates, with failure to complete required information resulting in bid rejection as non-responsive. The state reserves the right to increase or decrease quantities at the same unit prices provided, remove items as necessary for budgetary compliance, and adjust or delete bid alternates if total costs exceed budget constraints. Bidders must confirm they have examined the contract documents and site conditions, and agree to furnish all necessary plant, labor, equipment, and materials to complete the work in accordance with contract specifications. Items not explicitly listed in the bid schedule are considered incidental to the contractor's scope of work.
