About this file
This amendment provides additional requirements for a federal contract to replace an end-of-life HVAC unit. The amendment specifies that the crawlspaces associated with the project are considered confined spaces, and requires contractors to provide documentation of confined space entry training and equipment. Contractors must submit copies of their written confined space entry program, training records for entrants and supervisors, evidence that air monitoring equipment is properly calibrated, and specify that they will provide all necessary personal protective equipment and fulfill all entry roles. The related federal contract opportunity is solicitation number ISD-348339-SM from the Department of Energy's Fermi National Accelerator Laboratory to replace the HVAC unit identified as ITE 38.

ISD-348339-SM
Amendment 01
9/13/23
The purpose of this amendment is to:
1. Add information regarding the crawlspace. The crawlspaces are considered a confined space, the contractors must provide proof of confined entry training for each of their employees entering the crawlspace. Contractors must also provide their own air monitoring equipment while they’re examining the crawlspace.
• Before the subcontractors come on site (ideally sometime this week to allow me time to review), they will need to send:
i. A copy of their written confined spaces entry program
ii. Training records for potential entrants, attendants, and entry supervisors
iii. Evidence that all air monitoring equipment is properly calibrated within the calibration period specified by the subcontractor's program or manufacturer's instructions. This may be in the form of a calibration log, certification indicator on the instrument, or other means. (It is imperative that the equipment used by the subcontractor be capable of monitoring for the contaminants associated with the confined space to be entered.)
iv. It will be the subcontractor's responsibility to provide all their own personal protective equipment (PPE), such as lifelines, harnesses, respirators, tripods, ventilators, etc., as specified by the entry permit as well as air monitoring equipment
v. The contractor will fill all roles of entrant, attendant, and supervisor – that way they will just fill out their own permit and provide you with a copy of it at the end of the entry.
