About this file

This document is an Iran Free Procurement Certification Form required for vendors seeking to contract with the Commonwealth of Pennsylvania for goods or services worth $1,000,000 or more. The form outlines two options for vendors: certifying they are not on the list of persons engaged in investment activities in Iran, or demonstrating they have received an exemption from the certification requirement. The document cites Pennsylvania's Procurement Code Sections 3501-3506 and the penalties for providing false certifications.

The related state and local contract opportunity is for a Spray Foam Trailer, to be procured by Lebanon County in Pennsylvania. The form and certification requirements would apply to this procurement.
