About this file
This is an Invitation for Bid (IFB) form submitted to the State of Illinois Department of Human Services for maintenance, repairs, electrical testing, and lighting upgrades on three high-mast lighting poles at Chester Mental Health Center located at 1315 Lehmen Drive, Chester, Illinois. The procurement is designated for small business set-aside program vendors only. The document serves as the standardized bid submission form (version 25.3) that bidders must complete and return to be considered for this contract opportunity. The form requires bidders to confirm compliance with all solicitation requirements, acknowledge that failure to meet requirements constitutes grounds for disqualification, and submit all required documentation as outlined in the solicitation's Section A.1.
The IFB form provides bidders with the opportunity to claim applicable preferences under the Illinois Procurement Code, including Illinois Business Preference and Domestic Product Preference, provided the vendor submits the required affidavits and documentation. Bidders are permitted to take exceptions to standard terms and conditions by submitting a redlined version of the Standard Terms and Conditions document, which the State may accept or reject at its discretion. The form requires the authorized representative's signature and date to formalize the bid submission. Standard Terms and Conditions for any resulting contract are available through the Illinois Procurement Portal at https://cpo-general.illinois.gov/solicitation-and-contract-templates.html.
