About this file
This is an Invitation for Bid (IFB) document submitted to the State of Illinois Department of Human Services (DHS) for the procurement of Axis S1296 Rack 96 TB Network Video Recorders. The equipment is intended for installation at two facilities: the Elisabeth Ludeman Center located at 114 N. Orchard Drive, Park Forest, Illinois 60466, and the Samuel H. Shapiro Developmental Center located at 100 E. Jeffery Street, Kankakee, Illinois 60901. This is a rebid of a previously solicited contract identified as Bid B-52430. The document serves as the bidder's formal offer to perform in full compliance with the solicitation requirements, and bidders are required to complete and submit all documentation specified in Section A.1 of the solicitation to avoid disqualification.
The IFB form provides bidders the opportunity to claim applicable preferences under the Illinois Procurement Code, including Illinois Business Preference and Domestic Product Preference, with the vendor required to complete affidavits or designate products accordingly. Bidders may take exceptions to the Standard Terms and Conditions by submitting a redline version of those terms with their bid; however, the State reserves the right to accept or reject any proposed exceptions. The Standard Terms and Conditions, which will become part of any resulting contract, are available on the Illinois Central Procurement Office website at https://cpo-general.illinois.gov/solicitation-and-contract-templates.html. Specific pricing terms, quantities, response deadlines, and award information are not detailed in this particular document excerpt.
