About this file

This is an insurance requirements exhibit for Seminole County, Florida's Request for Proposal (RFP-5187-26) for Fire Department Physician Services. The procurement seeks comprehensive annual hands-on physicals for over 500 existing Fire Department employees, along with pre-employment physicals, return-to-work physicals, and respirator use evaluations compliant with NFPA Standard 1582. Services must be performed within 30 miles of Fire Department Headquarters in Sanford, Florida, with blood work collected at a central county location. Proposals are due electronically by 2:00 pm on March 25, 2026, with questions accepted until 5:00 pm on March 13, 2026. The initial contract term is three years with options for two successive one-year renewals and a possible additional one-year extension by mutual agreement, subject to a three-month probationary period allowing immediate termination by the County. Evaluation scoring allocates 25 points each to qualifications and certifications, experience, understanding and approach, and pricing.

All respondents must maintain comprehensive insurance coverage as outlined in Exhibit D, including Workers' Compensation with $1,000,000 employers' liability limits, Commercial General Liability with $1,000,000 per occurrence and $2,000,000 aggregate limits, Business Automobile Liability with $1,000,000 combined single limit, Professional Liability with $1,000,000 per occurrence and $2,000,000 aggregate limits, and Cyber Liability and Technology E&O insurance with $1,000,000 per occurrence and $2,000,000 aggregate limits. Seminole County must be named as an additional insured on applicable policies. Compensation is structured on a fixed-fee basis through written Purchase Orders, with monthly invoicing to the Director of County Comptroller's Office and payment processed under Florida's Prompt Payment Act. The specific budget or award value is not disclosed, and the County reserves the right to purchase any, all, or none of the requirements from awarded vendors, with price redeterminations permitted during renewal terms based on documented changes in wages, fuel, or materials.
