Credentialing Solution

PART 1 – DESCRIPTION

The Indian Health Service (IHS) has a requirement to obtain a commercial off the shelf (COTS) product that is an online, consistent, established and industry compliant resource that staff across IHS can benefit from to support quality medical staffing, credentialing, privileging and peer review activities. This resource shall provide a collection of continuously updated tools, best practices, articles, books, webinars, and compliance tips for credentialing, privileging and peer review topics developed by industry experts.

This solicitation is a Request for Quote (RFQ) using FAR 12 and FAR 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2025-

03. The North American Industry Classification System (NAICS) code for the proposed acquisition is 611430, Continuing Education Training, Online (though a website or mobile application).

3.1 Background

Currently, the Indian Health Service, does not have an ongoing, formal IHS Medical Staff Professional (MSP) training available to IHS MSP. There is currently one IHS HQ Credentialing Program Manager to field e-mails and call for consultation, technical assistance and training on standard credentialing and

privileging processes for approximately 75 new and veteran IHS MSP at 10 Area Offices and 105 federal facilities. The Office of Quality (OQ) Division of Quality Assurance and Patient Safety is committed to the IHS Agency Work Plan Patient Safety Priority of enhancing standardization of the credentialing system, which includes agency- wide credentialing and privileging training of its MSPs*.

The IHS needs to establish a training for new and veteran IHS MSPs in foundational industry and IHS agency credentialing and privileging process; however, developing and funding a formal training program will take significant planning funding, and time. IHS is seeking current, industry backed credentialing, privileging and peer review online resources to help partially fill this need by offering current industry related topics from industry experts on a variety of medical staffing, privileging, and peer review topics while the Agency policy and procedure manual is developed and software standardization continues.

*IHS MSPs are staff that are tasked at IHS facilities with preparing the medical staff credentialing and privileging files of licensed practitioners (LP) for approval by the Governing Body. This includes knowledge and skills of how to verify, assess, and document primary source verifications of required education, training, work history, insurance, health status, including specific LP credentials by specialties. In addition, MSPs must also understand and apply knowledge from medical staff bylaws, accreditation standards, and deferral laws as they prepare the credentialing and privileging file for review, recommendation and approval. These important tasks are why MSPs are known as the Gatekeepers of Patient Safety.

3.2 Statement of Work / Scope of Work

All services in this performance work statement shall be provided in accordance with provisions within this Performance Work Statement. The vendor will provide a ready to utilize commercial off the shelf (COTS) products and services to all IHS Operated facilities, including twenty four (24) hospitals, twenty six (26) clinics, eleven (11) IHS Area Offices and a HQ staff of five (5).

The following minimum requirements shall be applicable to all services furnished:

1. Shall provide the access electronically at the time of award via the internet.

2. Shall not require any additional downloads.

3. Shall allow for topics and keywords to be searchable.

4. Contents shall be written, published, developed by industry-backed experts. Industry-backed experts are indicated by credentials of the industry field, for credentialing and privileging, acceptable credentials are Certified Provider Credentialing Specialist (CPCS®) or Certified Professional Medical Services Management (CPMSM®), for medical staffing, governance, and peer review acceptable credentials are CPCS, CPMSM, MD, DOs, or other licensed independent medical professionals writing or developing in their field of specialty (Optometrist providing peer review article or tool on optometry). For legal medical staffing counsel, the experts should be a JD with experience in credentialing, privileging, medical staffing, peer review, and governance evidenced by nationally published books, speaking at the National Association of Medical Service Professional industry conferences or trainings.

3.3 Salient Characteristics / Minimum Requirements

3.3.1 Credentialing Resources

The product shall meet or exceed the minimum salient characteristics:

Shall have a minimum of one (1) weekly industry related articles, news, compliance strategies, and expert analysis published that provides history, background, updates, challenges, and opportunities related to medical staff credentialing, privileging, peer review, medical staff governance, state and federal regulations, and legal considerations to include historical published articles, news, compliance strategies and expert analysis for the past ten years.

Shall have a minimum of thirty five (35) Clinical Privilege White Papers to be available at the time of award to view and download to include regulations and standards for accrediting bodies (at a minimum for The Joint Commission and The Centers for Medicare and Medicaid, inclusive of the Accreditation Association for Ambulatory Health Care), to include current recommendations and positions of specialty medical boards, minimum criteria for education, training, and competency for initial and reappointment applicants that reflect best practices and expert research for a variety of physician, advance practice, and allied health specialties and subspecialties to include listing of core privileges and special non-core privileges or procedures for the following practice area:

Family Medicine, Pediatrics, OB/GYN, Emergency Medicine, Internal Medicine, Radiology, Anesthesiology, Pathology, Endocrinology, Rheumatology, Clinical Psychology, Psychiatry, Addiction Medicine, Addiction Psychiatry, Urgent Care Medicine, Physical Therapy, Child and Adolescent Psychiatry, Social Work, Pediatric Dentistry, Dentistry, Surgical First Assistants, Chiropractor, Respiratory Therapist, Occupational Therapist, Clinical Nurse Specialist, Nurse Midwife, Physician Assistant in Family Medicine, Physician Assistant in Emergency Medicine, Hospital Pharmacist, Optometrist, Nurse Practitioner in Family Medicine, Nurse Practitioner in Emergency Medicine, Hospitalist, Moderate Sedation, and Certified Registered Nurse Anesthetist.

Shall have a minimum of twenty (20) sample core clinical privilege forms to be available at the time of award to view and download to include Family Medicine, Emergency Medicine, Pediatrics, Internal Medicine, OB/GYN, Pathology, General Surgery, Radiology, Psychology, Psychiatry, Physician Assistant, Nurse Practitioner in Family Medicine, Nurse Practitioner in Emergency Medicine, Nurse Practitioner in Psychiatry, Registered Nurse First Assistant, Physical Therapy, Certified Nurse Midwife, Anesthesiology, Dental, and Pediatric Dentistry.

At minimum the following subjects: sample forms, language, checklists, tools, policies, and procedures should be available for viewing and download at the time of award: the aging physician; disruptive physician; professional misconduct; interview questionnaire for Medical Service Professionals (MSP), Medical Executive Committee, and Governing Body; ongoing professional practice evaluations for licensed practitioners; focused professional practice evaluations for licensed practitioners; peer review for allied health; prescreen, initial appointment, and reappointment checklists; position descriptions of medical staff leaders; bylaw language for a variety of medical staff topics; vaccination policy; physical health self-assessment; physician indicators of performance; credentials committee report; medical executive committee report;

governing body report; medical staff orientation checklist; disaster management credentialing;

emergency management credentialing; telemedicine credentialing; decision notification letters;

amending bylaws; meeting planning guide; temporary privileges; proctoring; residents; students;

fair hearing; credentialing red flags; credentialing audits; medical staff categories; medical staff statuses; confidentiality of medical staff records; provider enrollment; six core competencies; low and no volume practitioners, impaired physicians; dispute resolution; leave of absence, meeting agenda; meeting minutes; and privilege addendums.

Shall have a minimum of five (5) eBooks available at the time of award written by medical staff industry experts and professionals be available to be viewed unlimited during the contract period covering the following topics: Medical staff credentialing, clinical privileging, medical staff leadership and governance, peer review, quality, compliance, process improvement, leadership, overcoming medical staff challenges and negligent credentialing.

Shall have a minimum of twenty (20) on demand webinars (specific to medical staff credentialing and privileging, medical staff governance, medical staff accreditation and regulatory requirements, and medical staff peer review and quality topics) to be viewed unlimited during the contract period and available at the time of award.

Access to a source directory of schools, education programs, hospital medical staff offices, and specialty boards to identify credentialing contacts available at the time of award.

3.3.2 E- Learning: Credentials Verification Module

The resource shall include an e-learning credentials verification module with unlimited views during the contract period, and available at the time of award. Additionally, this module shall meet or exceed the following:

The module shall provide the differences of privileging and credentialing to MSPs who are new to their role or could use a refresher on the basics.

Shall include acceptable sources for credentials verification, the differences between verifying competency and previous affiliations, and the challenges of verifying professional references.

The module shall have an exam to test and document completion and proficiency and provide

National Association of Medical Service Professional CEUs.

The module shall be part of a learning management system that allows for tracking of completion and report generation to share attendee data back to IHS.

The modules shall be internet based and not require an additional download to install software.

3.3.3 E-Learning: Fundamentals of Credentialing Module

The resource shall include an e-learning fundamentals of credentialing module with unlimited views during the contract period and available at the time of award, at a minimum this module shall meet or exceed the following:

The modules shall provide a complete and full picture of what is meant by credentialing, privileging, and peer review, and how these three elements work in alignment to protect patients.

The module shall have an exam to test and document completion and proficiency and provide

National Association of Medical Service Professional CEUs.

The module shall be part of a learning management system that allows for tracking of completion and report generation to share attendee data back to IHS.

The modules shall be internet based and not require an additional download to install software.

3.3.4 Section 508 and Accessibility

This product must conform to the Revised Section 508 (https://www.access-board.gov/guidelines-and-standards/communications-and-it/about-the-ict-refresh/final-rule/text-of-the-standards-and-guidelines) and WCAG 2.0 Level AA standards as outlined below.

(a) Pursuant to Section 508 of the Rehabilitation Act of 1973 (29 U.S.C. 794d), as amended by the Workforce Investment Act of 1998, all electronic and information technology (EIT) supplies and services developed, acquired, or maintained under this contract or order must comply with the “Architectural and Transportation Barriers Compliance Board Electronic and Information Technology (EIT) Accessibility Standards” set forth by the Architectural and Transportation Barriers Compliance Board (also referred to as the “Access Board”) in 36 CFR part 1194. If it is determined by the Government that EIT supplies and services provided by the Contractor do not conform to the described accessibility standards in the contract, remediation of the supplies or services to the level of conformance specified in the contract will be the responsibility of the Contractor at its own expense.

Support Documentation Accessibility

• E602.1 General - Documentation that supports the use of ICT shall conform to 602.

Functional Performance Criteria

• 301.1 Scope - The requirements of Chapter 3 shall apply to ICT where required by 508 Chapter 2 (Scoping Requirements), 255 Chapter 2 (Scoping Requirements), and where otherwise referenced in any other chapter of the Revised 508 Standards or Revised 255 Guidelines.

• 302.1 Without Vision - Where a visual mode of operation is provided, ICT shall provide at least one mode of operation that does not require user vision.

• 302.2 With Limited Vision - Where a visual mode of operation is provided, ICT shall provide at least one mode of operation that enables users to make use of limited vision.

• 302.3 Without Perception of Color - Where a visual mode of operation is provided, ICT shall provide at least one visual mode of operation that does not require user perception of color.

• 302.4 Without Hearing - Where an audible mode of operation is provided, ICT shall provide at least one mode of operation that does not require user hearing.

• 302.5 With Limited Hearing - Where an audible mode of operation is provided, ICT shall provide at least one mode of operation that enables users to make use of limited hearing.

• 302.6 Without Speech - Where speech is used for input, control, or operation, ICT shall provide at least one mode of operation that does not require user speech.

• 302.7 With Limited Manipulation - Where a manual mode of operation is provided, ICT shall provide at least one mode of operation that does not require fine motor control or simultaneous manual operations.

• 302.8 With Limited Reach and Strength - Where a manual mode of operation is provided, ICT shall provide at least one mode of operation that is operable with limited reach and limited strength.

• 302.9 With Limited Language, Cognitive, and Learning Abilities - ICT shall provide features making its use by individuals with limited cognitive, language, and learning abilities simpler and easier.

(d) In the event of a modification(s) to this contract or order, which adds new EIT supplies or services or revises the type of, or specifications for, supplies or services, the Contracting Officer may require that the contractor submit a completed HHS Section 508 Product Assessment Template and any other additional information necessary to assist the Government in determining that the EIT supplies or services conform to Section 508 accessibility standards.

Instructions for documenting accessibility via the HHS Section 508 Product Assessment Template may be found under Section 508 policy on the HHS website: (http://www.hhs.gov/web/508). If it is determined by the Government that EIT supplies and services provided by the Contractor do not conform to the described accessibility standards in the contract, remediation of the supplies or services to the level of conformance specified in the contract will be the responsibility of the Contractor at its own expense.

(e) If this is an Indefinite Delivery contract, a Blanket Purchase Agreement or a Basic Ordering Agreement, the task/delivery order requests that include EIT supplies or services will define the specifications and accessibility standards for the order. In those cases, the Contractor may be required to provide a completed HHS Section 508 Product Assessment Template and any other additional information necessary to assist the Government in determining that the EIT supplies or services conform to Section 508 accessibility standards. Instructions for documenting accessibility via the HHS Section 508 Product Assessment Template may be found at http://www.hhs.gov/web/508. If it is determined by the Government that EIT supplies and services provided by the Contractor do not conform to the described accessibility standards in the provided documentation, remediation of the supplies or services to the level of conformance specified in the contract will be the responsibility of the Contractor at its own expense.

3.5 Contract Administration

Contracting Officer’s Representative (COR)

The COR is responsible for the following as required by this order: (1) monitoring the Contractor's technical progress, including the surveillance and assessment of performance and recommending to the Contracting Officer changes in requirements; (2) interpreting the Statement of Work and any other technical performance requirements; (3) performing technical evaluations; (4) performing technical inspections and acceptances; and (5) assisting in the resolution of technical problems encountered during performance.

3.6 Payment

The Government will pay the Contractor only each respective deliverable’s firm-fixed-price delineated in the table in Subpart 2 above, upon successful completion, as determined by the COR, and upon submission of a proper invoice. Invoicing under this order shall be processed through the Invoice Payment Platform (IPP), located at www.ipp.gov .

IPP is a secure, web-based electronic invoicing system provided by the U.S. Department of the Treasury's Bureau of the Fiscal Service, in partnership with the Federal Reserve Bank of St. Louis (FRSTL). The Office of Management and Budget (OMB) M-15-19 Memorandum "Improving Government Efficiency and Saving Taxpayer Dollars Through Electronic Invoicing" directs Federal agencies to adopt electronic invoicing as the primary means to disburse payment to vendors.

If your organization is not currently utilizing IPP, following award of an order, your organization will be automatically enrolled by IHS. Your company’s IPP POC will be determined by the information listed in your SAM.gov account, and will receive two e-mails from the Bureau of Fiscal Service, one with a username and one with a temporary password. (a) Definitions. As used in this clause—

Payment request means a bill, voucher, invoice, or request for contract financing payment with associated supporting documentation. The payment request must comply with the requirements identified in FAR 32.905(b), "Content of Invoices" and the applicable Payment clause included in this contract.

(b) Except as provided in paragraph (c) of this clause, the Contractor shall submit payment requests electronically using the Department of Treasury Invoice Processing Platform (IPP) or successor system.

(c) The Contractor may submit payment requests using other than IPP only when the Contracting Officer authorizes alternate procedures in writing in accordance with HHS procedures.
