About this file
This is an Indian Economic Enterprise (IEE) Representation Form from the Indian Health Service (IHS), Department of Health & Human Services, used to certify eligibility under the Buy Indian Act (25 U.S.C. 47). The form is specifically associated with a pre-solicitation for janitorial services at the Catawba Service Unit Health Center under NAICS code 561720.
The form requires offerors to self-certify that they meet IEE eligibility requirements, which include at least 51% Indian ownership, receipt of at least 51% of contract earnings by Indian owners, and Indian control of management and daily operations. Offerors must maintain eligibility throughout the offer period, contract award, and full contract performance period. The form requires specific information including the name of the Federally Recognized Tribal Entity, business name, DUNS number, and certification by the 51% owner. The document emphasizes that false certifications are punishable under various U.S. codes and that successful offerors must be registered in the System for Award Management (SAM).
