About this file

This is an Invitation for Bid (IFB) Offer Form submitted to the State of Illinois for a used tire removal service covering regions 3 and 4. The document serves as the official bid submission form that vendors must complete and submit in accordance with the solicitation requirements outlined in section A.1 of the solicitation document. Bidders are required to ensure all mandatory documents are included with their submission, as failure to meet all requirements may result in disqualification. The form requires bidders to confirm their understanding of compliance obligations and submit a signed offer to perform services in full compliance with the subject solicitation terms.

The form provides bidders the opportunity to claim applicable preferences under the Illinois Procurement Code, including the Illinois Business Preference and Domestic Product Preference. Bidders claiming the Illinois Business Preference must submit a completed affidavit, while those claiming the Domestic Product Preference must designate "Domestic Product" in the alternate description field for qualifying line items. Additionally, the form allows bidders to take exceptions to Standard Terms and Conditions by submitting a redlined version of the applicable terms, though the State reserves the right to accept or reject any proposed exceptions. All resulting contracts will incorporate the Standard Terms and Conditions available on the CPO General Illinois website.
