IFB Offer to the State of Illinois for D5 Rest Areas Septic Systems

This is an Invitation for Bid (IFB) offer submission document to the State of Illinois Department of Transportation (IDOT) for septic system services at four rest areas located in IDOT District 5. The solicitation is identified as B-53916 and represents a rebid procurement. The document serves as the formal bid submission form where vendors provide their offers in compliance with the solicitation requirements outlined in section A.1, with failure to meet all stated requirements resulting in disqualification.

The bid form includes provisions for vendors to claim applicable Illinois business preferences, including the Illinois Business Preference and Domestic Product Preference, with vendors required to submit supporting affidavits or documentation as applicable. Bidders may take exceptions to standard terms and conditions by submitting a redline version of the State's Standard Terms and Conditions, though the State reserves the right to accept or reject any proposed exceptions. The form references the State of Illinois Procurement Code preferences and directs bidders to review standard contract terms available on the Illinois Chief Procurement Office website before submission.
