About this file

This is an Exhibit A offeror information form for the State of Hawaii Department of Education's Invitation for Bid (IFB D27-019) soliciting maintenance and repairs services for air conditioning and ventilating equipment on the island of Oahu. Offerors must provide completed Exhibit A documentation within three working days of the STATE's request and demonstrate at least five consecutive years of experience in providing maintenance and repairs for HVAC equipment. The contract is organized into three service groups: Group A covering the Central District, Group B covering Honolulu and Windward Districts, and Group C covering exhaust and ventilating fans. All work will be performed on the island of Oahu with service technicians assigned to specific groups based on specialized training requirements.

Offerors must maintain an office and service facility on Oahu with direct telephone access during business hours (7:45 a.m. to 4:30 p.m. HST, Monday through Friday) and designate one to two employees as points of contact for the STATE. A minimum of four to six qualified service technicians must be assigned per group, with specific technical requirements including reciprocating and/or screw chiller training and inverter technology training for Groups A and B. Offerors must provide three references from companies or government agencies (excluding the Hawaii State Department of Education) who can attest to service quality and reliability. Service technicians currently assigned to existing STATE HVAC service contracts are ineligible, and each technician may only be assigned to one group.
