About this file

This is a bid terms and conditions document issued by the State of Maine, Division of Purchases, for Humanscale Ergonomic Products and Accessories. The State is seeking bids to furnish a portion of normal requirements for Humanscale ergonomic products and accessories, including items from the "Ergo Tools" and "Monitor Arm" price guides as specified in an attached spreadsheet. The contract will run for one year beginning approximately September 1, 2008, and ending August 31, 2009, with possible renewal options for two additional one-year periods. All pricing information and manufacturer documentation must be completed on the provided spreadsheet and uploaded to the AdvantageME response system, with unit costs listed as $0.00 on the bid itself and actual pricing included in the spreadsheet. Bidders must provide documentation on delivery times and procedures, including the number of delivery days from receipt of order.

Prices quoted must remain firm throughout the contract period and shall include delivery and installation costs at the point of delivery. The estimated yearly contract value is approximately $90,000, though the actual value will reflect all quantities ordered by state agencies during the contract term. Price increases are permitted only when supported by revised manufacturer price lists or notices documenting general industry changes, subject to State approval; price reductions may occur at any time. The State reserves the right to seek additional discounts or contract separately for large purchases. Bids will be evaluated on an "all or none" basis, considering price, warranty, delivery timeframes, and other factors. Vendors must accept the State of Maine Procurement Card as payment with no surcharges allowed. Separate invoices must be issued for each order, referencing the contract number and vendor customer number, and should be issued within 30 days of delivery. Bidders must indicate in their response whether they will extend contract pricing to political subdivisions and Maine school districts. The primary contact for this bid is Kathy Paquette at the Division of Purchases.

All contract orders (oral or written) dated prior to the effective date of the increase shall be filled at the old price(s). INVOICES: Separate invoices are required for each order number. All invoices must reference Contract Number and Vendor Customer Number, failure to do so could cause lengthy delays of payment of invoices. All invoices should be issued within 30 days of delivery and paid in a timely manner agreed upon between the using agency and the successful vendor.

TAXES: The State is exempt from the payment of excise taxes. Exemption certificates will be furnished upon request.
