About this file

This is a blank HUD Form 2516 Contract/Subcontract Award Form used by the U.S. Department of Housing and Urban Development to report contract and subcontract activities across multiple HUD programs including Community Development Block Grants (CDBG), Public Housing Authorities (PHAs), Indian Housing Authorities (IHAs), and multifamily housing programs. The form is completed by grantees, project owners, developers, sponsors, builders, and agencies to document contracts and subcontracts valued at $10,000 or more executed during specified reporting periods. The reporting entity identified in the template is Palm Beach County, located at 100 Australian Avenue, Suite 500, West Palm Beach, Florida 33406, with a contact person reachable at (561) 233-3634. The form covers the reporting period of October 1, 2019 through September 30, 2020. Two copies of the completed report must be submitted to the local HUD Office within ten days after the end of the applicable reporting period.

The form captures detailed information about prime contractors and subcontractors, including employer identification numbers, contract amounts, type of trade codes, contractor business racial/ethnic composition, woman-owned business status, and Section 3 compliance designations. Section 3 requirements mandate that employment and economic opportunities generated by HUD financial assistance be directed toward low- and very low-income persons, with specific income thresholds defined as families earning no more than 80 percent of the area median income for low-income persons and 50 percent for very low-income persons. The form also tracks minority business enterprise (MBE) goals and performance, with racial/ethnic codes distinguishing among White Americans, Black Americans, Hispanic Americans, Native Americans, Asian/Pacific Americans, and other categories. This blank template serves as the standard reporting instrument for all eligible HUD-funded projects within the identified jurisdictions.
