About this file
This is a Certification for a Drug-Free Workplace form (HUD-50070) issued by the U.S. Department of Housing and Urban Development.
The form requires applicants receiving federal grant funding from HUD to certify compliance with drug-free workplace requirements. Applicants must commit to providing a drug-free workplace by: publishing a statement notifying employees that unlawful manufacture, distribution, dispensing, possession, or use of controlled substances is prohibited and specifying actions against violators; establishing an ongoing drug-free awareness program informing employees about drug abuse dangers, the organization's drug-free policy, available counseling and rehabilitation programs, and penalties for violations; requiring each employee to receive a copy of the required statement as a condition of employment; and notifying employees that compliance with the drug-free workplace requirement is a condition of employment. Additionally, applicants must notify the agency in writing within ten calendar days of receiving notice of any employee conviction for a drug violation occurring in the workplace, and must take appropriate personnel action within 30 calendar days of such notice, including either terminating the employee or requiring participation in a drug abuse assistance or rehabilitation program. Applicants must also list all work performance sites on separate pages, including street addresses, cities, counties, states, and zip codes. The form requires certification under penalty of perjury by an authorized official, with an OMB control number 2501-0044 and expiration date of 2/28/2027.
