MEMORANDUM FOR ALL PROSPECTIVE QUOTERS

SUBJECT: Request for Quotation (RFQ) HTC711-22-Q-R010, Air Ambulance Services

1. The United States Transportation Command (USTRANSCOM) Specialized Transportation and Support Division (TCAQ-R) intends to establish Blanket Purchase Agreements (BPAs) for air ambulance service with a five-year period of performance. The aircraft utilized shall be contractor owned and operated aircraft, configured to air ambulance standards. The BPAs will be for service from Outside the Continental United States (OCONUS) to the Continental United States (CONUS) and within CONUS for the movement of Department of Defense (DoD) eligible patients. Special requirements may be offered on a case-by-case basis. In the future, service areas may be expanded to include within OCONUS Geographic Combatant Commands (GCCs). The Area of Responsibilities (AORs) are defined at the following website: https://www.defense.gov/About/combatant-commands/.

2. The BPAs will be used to supplement military capability. There is no guarantee any service will be purchased by the Government. A best value decision, outlined in Attachment 3, Additional BPA Information, will be made prior to issuing each BPA call for service.

3. Medical crews must be accredited by the Commission on Accreditation of Medical Transport Systems (CAMTS) and/or the National Accreditation Alliance of Medical Transport Applications (NAAMTA).

The air carrier must also be DoD-approved by the Commercial Airlift Review Board (CARB) in accordance with 32 Code of Federal Regulation (CFR) Part 861. All carriers shall comply with the requirements included in Attachment 3, Additional BPA Information.

4. Notwithstanding the requirements in paragraph 3 above, the following considerations apply:

a. Conditional BPA award notices may be initially sent to quoters who are DoD-approved air carriers and have initiated the process to receive CAMTS and/or NAAMTA accreditation. In this case, the carrier will only be eligible for missions if needed to save “life, limb, or eyesight” until the accreditation standard is attained. If the medical accreditation is not achieved within twelve months of the closing date specified in paragraph 6 below, the conditional award notice will be withdrawn.

b. A quoter that is either a CAMTS and/or NAAMTA accredited air carrier, but is not a DoD-approved air carrier, will receive a letter of intent. Upon notice of a letter of intent, the quoter shall immediately pursue DoD approval. However, if a quoter is unable to achieve DoD approval within twelve months of the issuance of the letter of intent, an award will not be made unless the Contracting Officer approves an extension.

c. Upon receipt of the required credentials (i.e., DoD approval and CAMTS and/or NAAMTA accreditation), the letter of intent or conditional award will be transitioned to a full award. Quoters that do not have either DoD approval or a medical accreditation (by CAMTS or NAAMTA) will not be considered for award and will be removed from the competition upon receipt of the quote.

5. Medical staffing and retrofitting of medical equipment shall be commensurate with patient requirements. For instance, patients in stable condition may only require one medical professional of at least an Aeromedical Crew Member (ACM) Level-1. In contrast, specialized missions may require ACM Level-4 staffing. A list of definitions is provided in Attachment 3.

6. To be considered for BPA award, the following information must be submitted via email to the Points of Contact (POCs) listed in paragraph 9 no later than 11:59 AM (CT) on 26 July 2022.

a. A Price List (Attachment 1). All price lists must be signed, dated, and indicate the company name and address. Round-trip missions and point-to-point missions are defined in Attachment 3.

Additional potential reimbursable expenses may be added to the price list by an attachment. All prices shall be submitted in US Dollars (USD) with two decimal places only. Award and payment shall be made in USD.

b. A List of Aircraft (Attachment 2).

c. Completed fill-ins for the provisions located in Attachment 5, List of Provisions. Specifically, IAW Federal Acquisition Regulation (FAR) 4.1102(a), Representations and Certifications shall be submitted online using https://www.sam.gov where available OR may be submitted with your quote. The following provisions require your input and must be included with your quote if they are not in your SAM record.

• FAR 52.204-24, Representation Regarding Certain Telecommunication and Video Surveillance Services or Equipment (Oct 2020) (NOTE: FAR 52.204-24 only needs to be completed if FAR 52.204-26 is not completed in SAM.gov, or if the quoter represents it “will” provide or “does” use covered telecommunications equipment in FAR 52.204-26.)

• FAR 52.209-7, Information Regarding Responsibility Matters (Oct 2018)

• FAR 52.212-3, Offeror Reps & Certs – Commercial Items (Nov 2021) with Alternate I

• DFARS 252.204-7017, Prohibition on the Acquisition of Covered Defense Telecommunications

Equipment or Services—Representation (May 2021) (NOTE: DFARS 252.204-7017 only needs to be completed if DFARS 252.204-7016 is not completed in SAM.gov, or if the quoter represents it “does” provide covered defense telecommunications equipment or services in

DFARS 252.204-7016.)

d. The information required in Attachment 3, paragraph 9b, entitled “Air Carrier Insurance Requirements.” and the information required by FAR 52.237-7, Indemnification and Medical Liability Insurance, included in Attachment 4, List of Clauses.

e. For air carriers with CAMTS and/or NAAMTA accreditation, documentation from the accrediting source that verifies the accreditation.

f. For DoD-approved air carriers without CAMTS and/or NAAMTA accreditation, documentation from the accrediting source that verifies the process to become accredited has been initiated.

g. For CAMTS and/or NAAMTA accredited air carriers without DoD approval, a completed DoD Statement of Intent, AMC Form 207 (Attachment 6).

h. FAA/Civil Aviation Authority (CAA) certification, OPSPEC A003, to operate under Instrument Flight Rules (IFR) and Visual Flight Rules (VFR) conditions and rules.

i. Completed Attachment 7, Basic Eligibility, which denotes whether the quoter meets the basic eligibility requirements to be considered for award.

7. Attachment 4, List of Clauses, includes FAR, Defense Federal Acquisition Regulation Supplement (DFARS), and USTRANSCOM Federal Acquisition Regulation Supplement (TRANSFARS) clauses that will be incorporated into the resulting BPA. In addition, Attachment 5, List of Provisions, includes FAR and DFARS provisions applicable to this solicitation. The wage determination applicable to this requirement is included at Attachment 8, Wage Determination.

8. BPA Evaluation

a. This acquisition is being conducted under FAR Part 13, Simplified Acquisition Procedures.

(1) Contract Documentation. Contractors must satisfy the documentation requirements specified in paragraph 6 to be eligible for award.

(2) Price. Quotes received must be determined to be fair and reasonable to be eligible for award.

In accordance with FAR 13.106-3(a)(1), it is anticipated this determination will be made based on competitive quotations received in response to the solicitation.

b. BPAs will be awarded to all eligible air carriers with prices determined to be fair and reasonable.

9. USTRANSCOM Contracting Office POCs listed below are the exclusive POCs for this acquisition.

No information concerning this RFQ or requests for clarification will be provided in response to quoter-initiated telephone calls. All such requests must be made in writing and submitted to the e-mail addresses listed above. Written inquiries will be answered in writing and provided to all quoters via SAM.gov. The deadline for receipt of any questions regarding this RFQ is 10 calendar days prior to the due date for quoters as specified in the RFQ or its amendments. Any questions received after that time may not be answered.

10. Acknowledgment of Solicitation Amendments. Any solicitation changes will be made via amendment. All amendments to the solicitation shall be acknowledged by the quoter by signing and submitting a copy of the amendment(s) with the quotation.
