Appendix 6.4 U.S. Government Sponsored FMS Shipments
1. India. The U.S. Government-sponsored shipper will be responsible for preparing the documentation/information listed below. U.S. Government-sponsored shipments will be executed under Department of State Form DSP 94, Authority to Export Defense Articles Sold under the FMS Program.
This is Repair and Return (R&R) material. The material has previously exported the U.S. on a DSP-94.
R&R material shall be exported from India under the provision of 22 CFR 123.4 (Exemptions for FMS).
1.1. DD Form 1149, Requisition and Invoice/Shipping Document or DD1384-1a Military Standard Requisitioning and Issue Procedures (MILSTRIP) Document
1.2. Export Security Declaration Customs Release Letter
1.3. Commercial Invoice
1.4. Harmonized Code for Parts; this code is located in Federal Logistics Database (FEDLOG)/Web Federal Logistics Information Service (FLIS) also referred to as Schedule B Number
1.5. Bill of Entry Number
1.6. Airway Bill and Bill of Lading
1.7. FMS Case Number
1.8. Export Authorization Letter
1.9. Exemption Letter from the Ministry of Finance
1.10. Harmonized Tariff Schedule
1.11. Importer’s Declaration Form
2. Greece. The U.S. Government-sponsored shipper will be responsible for preparing the documentation/information listed below. U.S. Government-sponsored shipments will be executed under Department of State Form DSP 94, Authority to Export Defense Articles Sold under the Foreign Military Sales Program. This is Repair and Return (R&R) material. The material has previously exported the United States on a DSP-94. R&R material shall be exported from Greece under the provision of 22 CFR 123.4 (Exemptions for FMS).
