About this file

This is a Past Performance Questionnaire (PPQ) template used by the Defense Health Agency (DHA) Western Markets Contracting Division to evaluate contractor past performance on U.S. Government contracts. The form is designed to be completed by government agencies or commercial companies who have worked with the contractor being evaluated and must be submitted no later than 14 August 2026, either directly to the DHA at jojie.n.urrete.civ@health.mil or through the offeror.

The questionnaire requires evaluators to provide information about specific contracts including contract number, delivery/task order number, project title, location, award date, completion date, contract value, and whether the offeror performed as prime contractor or subcontractor. Evaluators must rate the offeror on 12 performance criteria using standardized ratings: "E" (Exceptional), "VG" (Very Good), "S" (Satisfactory), "M" (Marginal), "U" (Unacceptable), and "NA" (Not Applicable). The rated categories include relationship with client's contract team, subcontractor management, employee hiring and retention, corporate management and integrity, quality of work and quality control, schedule performance, cost control, labor standards compliance, safety standards compliance, and timely documentation submission. The form also includes yes/no questions regarding cure notices or terminations, contractor problem-solving ability, willingness to award future contracts, customer satisfaction, and opportunity for the contractor to respond to negative comments. The evaluator must provide an overall rating and signature, and evaluators' names will not be disclosed to contractors per Federal Acquisition Regulation requirements.
