About this file
This is a Past Performance Questionnaire (PPQ) template issued by the Defense Health Agency (DHA) Healthcare Contracting Division West to solicit feedback on contractor performance. The form is designed to be completed by references or past clients and returned to april.l.delmendo.civ@health.mil no later than 10:00 a.m. Pacific Time on Tuesday, September 08, 2026. Alternatively, offerors may submit an official government performance evaluation in lieu of completing the form. The government reserves the right to verify all information provided.
The questionnaire evaluates contractors across 19 rating categories using a standardized six-point scale: "E" (Exceptional—greatly exceeded requirements), "VG" (Very Good—exceeded requirements), "S" (Satisfactory—met requirements), "M" (Marginal—met minimum with some deficiencies), "U" (Unacceptable—poor performance), and "NA" (Not Applicable). Evaluation areas include client relationship management, subcontractor coordination, employee hiring and retention, corporate integrity, work quality, quality control, schedule performance, cost control, labor compliance, safety compliance, and documentation timeliness. The form also includes yes/no questions regarding disciplinary actions, problem-solving capability, likelihood of future contract award, customer satisfaction, and opportunities for contractor response. Responses to "M" or "U" ratings require supplemental explanation. The form concludes with an overall rating selection and evaluator signature line. Per FAR 15.306(e)(4), evaluator identities remain confidential, though contractors may respond to adverse remarks under FAR requirements.
