About this file

This is a Past Performance Questionnaire (PPQ) template issued by the Defense Health Agency (DHA) Healthcare Contracting Division West for evaluating contractor past performance on federal contracts. The form is designed to collect structured feedback from previous clients or customers regarding an offeror's performance on specific contracts or task orders.

The questionnaire captures offeror and project information including contract number, delivery/task order number, contract title, location, award date, completion date, contract value, and whether the offeror performed as a prime contractor or subcontractor. Evaluators provide ratings across 12 performance categories using a standardized five-point scale: "E" (Exceptional), "VG" (Very Good), "S" (Satisfactory), "M" (Marginal), and "U" (Unacceptable), with "NA" available when information is not applicable. Performance categories assessed include client relationship management, subcontractor coordination, staffing and retention, corporate management and integrity, quality of work and quality control, schedule performance, cost control, labor compliance, safety compliance, and timely documentation submission. Additional yes/no questions address whether cure notices or termination actions were issued, whether the contractor resolved performance problems independently, contractor willingness to award future contracts, customer satisfaction, and opportunity for the contractor to respond to negative ratings. Evaluators must provide supporting information for any Marginal or Unacceptable ratings and provide an overall firm rating. Completed forms should be returned to april.l.delmendo.civ@health.mil no later than 10:00 a.m. Pacific Time, Tuesday, August 25, 2026. The government reserves the right to verify all information provided, and per FAR 15.306(e)(4), evaluator names will not be disclosed to contractors.
