About this file
This is a Past Performance Questionnaire (PPQ) template issued by the Defense Health Agency (DHA) Healthcare Contracting Division West for evaluating contractor past performance on federal contracts. The form is designed to be completed by references (government agencies or commercial companies) who have directly worked with an offeror on a previous contract and should be submitted by August 11, 2026 at 10:00 a.m. Pacific Time to april.l.delmendo.civ@health.mil, though offerors are encouraged to submit completed questionnaires with their quotes.
The questionnaire requires evaluators to rate contractor performance across 12 specific criteria using a standardized five-point scale: "E" (Exceptional—greatly exceeded requirements), "VG" (Very Good—exceeded requirements), "S" (Satisfactory—met requirements), "M" (Marginal—met minimum requirements with material deficiencies), and "U" (Unacceptable—poor performance). Rating criteria include relationship with client contract team, subcontractor management, employee staffing and retention, corporate integrity, work quality, quality control, schedule performance, cost control, labor compliance, safety compliance, and documentation submission timeliness. The form also includes yes/no questions regarding disciplinary actions, problem-solving capability, contractor willingness to re-award, customer satisfaction, and whether the firm was given opportunity to respond to negative comments. Evaluators must provide an overall firm rating and may offer supporting comments and remarks. The government reserves the right to verify all information and will not disclose the names of individuals providing reference information per FAR 15.306(e)(4).
