Questions and Answers - HT940826QE006

1. I need the serial #'s for the WRO's and for the CWP. The serial numbers given are not serial numbers.

A: Serial number listed in attachment 1 have been verified as correct.

CWP:1331350.

WRO's:1339028; 1339026; 1339025; 1407294

2. When was the last yearly Preventative Maintenance performed? This would include a PM Kit. If this was never done, when was the system installed?

A: CWP: 1331350. (Apr 26)

WRO's: 1339028 (Apr 26);1339026 (Apr 26); 1339025 (Apr 26); 1407294 (Oct 25)

If the question is if the contractor provides the PM kits, per the PWS the contractor provides everything required to maintain and service the equipment, this would include PM kits, test strips, and any associated supplies required to complete the task.

3. When can I come for a Site Survey? This will dictate some of the pricing, so it is necessary and critical for our bid.

A: Any Tuesday or Thursday, coordination with Service Contract Management, Medical Equipment Repair Center, and the Dialysis unit must be coordinated in advance of visit to ensure no interruption of patient care, escort availability and base access is arranged. Contact POC: r.l.vansittert.civ@health.mil

Patient treatment days are Mon, Wen, Fri and the unit would prefer not to have site visits on treatment days if at all possible.

-Chemical cleaning or membrane maintenance requirements?

-Carbon filter tank servicing and media replacement expectations?

A: IAW US, State, and Local laws and regulations, Joint Commission requirements, and Association for the Advancement of Medical Instrumentation (AAMI) standards for critical water systems, these are a Medical/Dialysis system

6. Whether consumables (filters, carbon media, cleaning agents, test kits) are to be included in the contractor's scope?

A: Yes, Medical/Dialysis usage.

9. Availability of system history, maintenance logs, or performance data?
