For items the contractor cannot submit by these means, the contractor shall deliver them to the DHA designated point of contact at:
Defense Health Agency, Clinical Support Division, Health Systems Performance Branch, 7700 Arlington Boulevard, Suite 5101, Falls Church, VA 22042-5101.

4.0 NOTICE REGARDING LATE DELIVERY:
In the event the contractor anticipates difficulty in complying with the task order schedule, the contractor shall immediately notify the Contracting Officer’s Representative (COR) in writing, giving pertinent details, including the date by which it expects to make delivery. This notification shall be informational only in character and COR’s receipt of the notice shall not be construed as a waiver by the Government of any contract delivery schedule, or any rights or remedies provided by law or under this contract.
5.0 EXTENSIONS: The contractor shall request in writing an extension of due dates for a deliverable under this contract to the COR prior to the due date. Requests shall include a detailed explanation of the circumstances justifying the extension, the date by which delivery can occur, and the justification for that date. In reviewing a request for extension, the COR shall consider whether there were circumstances beyond the foreseeable control of the contractor that prevented timely delivery and recommend approval or disapproval to the CO. Failure to adequately manage workload shall not be an acceptable basis for an extension. The contractor cannot assume an extension will automatically be granted.
6.0 REPORTS AND PLANS:
6.1 Unless otherwise specified, contractors shall electronically submit all deliverables in a format approved by the Government including, but not limited to, Microsoft Office Excel, Word, PowerPoint, and PDF. Unless otherwise specified, the contractor shall submit all deliverables via the PAT in the DHA E-commerce Extranet (https://tma-extranet.csd.disa.mil/logon/privacystatement.cfm).
