About this file
This document is a defect distribution and escape rate report for software programs covering the period from June 2025 to April 2026.
The report presents quality metrics for three programs: JOMIS, DHMSM, and EIDS. JOMIS recorded 1,076 total defects with an 84% escape rate, distributed across severity levels 1-5 with 85 Sev-1, 185 Sev-2, 94 Sev-3, 315 Sev-4, and 4 Sev-5 issues, plus 393 unadjudicated defects. DHMSM had a significantly higher defect count of 1,564 total issues with a 34% escape rate, comprised of 17 Sev-1, 176 Sev-2, 142 Sev-3, 265 Sev-4, 8 Sev-5, 950 unadjudicated, and 6 unassigned defects. EIDS showed minimal defect activity with only 2 Sev-3 issues recorded and no applicable escape rate due to partial coverage limitations. The escape rate metric tracks severity 1 and 2 issues detected during Development, Test & Evaluation (DT&E) based on detection source rather than release cycle. Unadjudicated bugs are tracked with initial severity assignments pending final adjudication through the end-user community defect review process. EIDS data limitations stem from its non-use of DT&E systems for over-the-shoulder testing activities.

1 Unadjudicated bugs are logged in the system with an initial severity. Upon adjudication through the end-user community defect review process, severity will be updated to its final state.
2 Prime mission vendor escape rate is tracked based on detection source (vendor to DT&E), not by release cycle. Scope for escape rate is severity 1 and 2 issues.
3 Sev 1+2 is the count of severity 1 and severity 2 issues detected at DT&E, which is the scope of the escape rate calculation (see footnote 2).
4 EIDS defect data reflects partial coverage, as EIDS does not use DT&E systems for over-the-shoulder testing.
