About this file
This is a Combined Synopsis/Solicitation for a Request for Offers (RFO) for commercial items issued by the Department of Defense. The solicitation HT001426QE027 seeks two Phacoemulsification Units for ophthalmic surgical use. This unrestricted competition is organized under NAICS code 339115 (Ophthalmic Goods Manufacturing) with a size standard of 1,000 employees. The requirement specifies firm fixed pricing with a standard delivery schedule of 60 days after contract award. Delivery is required on or before October 30, 2026, to ACH Blanchfield Campbell in Fort Campbell, Kentucky. Offers must be submitted via email to Contract Specialist Justin Balster (justin.balster2.civ@health.mil) or Contracting Officer Lorena Cruz (lorena.cruz@health.mil) no later than 4:00 PM Eastern Standard Time on September 1, 2026.
Offerors must provide unit pricing through the included pricing table, a technical description detailing compliance with stated requirements in the Statement of Need, and a statement of agreement with all solicitation terms and conditions. All offerors must be actively registered with the System for Award Management (SAM) prior to award. The evaluation will use a lowest price technically acceptable (LPTA) approach, with technical capability evaluated first—any quote receiving an "Unacceptable" technical rating will be eliminated from consideration—followed by price evaluation for all acceptable quotes. The solicitation incorporates multiple FAR and DFARS clauses addressing compliance requirements including small business representations, cost control, unique item identification, covered defense telecommunications equipment restrictions, Buy American provisions, and various representations regarding tax liability, foreign procurement taxation, and prohibited business practices. Inspection and acceptance will occur at both the contractor facility and the destination, with specific DoDAAC codes HT0783 (inspection) and HT0014 (administration) assigned for contract administration purposes.

The Government may use various price analysis techniques and procedures to ensure a fair and reasonable price. Some of those techniques are, but not limited to, comparison of quoted prices received in response to the solicitation and/or comparison of quoted prices with the Independent Government Estimate. Price must be determined to be fair and reasonable.

Before the quote's specified expiration time, the Government may accept a quote (or part of a quote), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

Price
An offerer must receive a rating of Acceptable for Technical to receive consideration for award. Price is the determining factor for all Acceptable quotes.
