Exhibit 11B Procedures for lost or stolen PIV card

FPS Instructions when completing the Certification of Loss, Theft, Damage or Compromise of Access Control Card Form as it applies to HSPD-12 PIV cards.

Lost/Stolen Cards The following steps are taken when a cardholder loses or has their PIV card stolen. It is the responsibility of the cardholder to ensure the following is accomplished as soon as possible after realizing their credential PIV card is missing.

1. In the event of loss, theft, or compromise of credentials, the cardholder responsible for the PIV card, or the employee discovering the condition will take the following steps.

2. The cardholder will take immediate action to effect recovery of the lost or stolen

PIV card, and obtain all available information concerning the loss, theft, or compromise of the PIV card.

3. The incident must be reported to the Federal Protective Service. An incident report (Form 3155) will be completed by the responding officer

4. The cardholder will notify (within 24 hours) their supervisor and/or Contracting Officer’s Technical Representative (COTR) of their contract.

5. The Supervisor or COTR will then contact the HSPD-12 Program Office as soon as they are informed via email. They will then provide written notification of the theft or loss as well as the following information

• Approximate time and date the PIV card was discovered missing

• Addresses of buildings that the cardholder had access to.

• A copy of the Incident Report or case number assigned

• A completed copy of the Certification of Loss, Theft, Damage or

Compromise of Access Control Card Form.

6. The HSPD-12 Program Office will then immediately revoke the PIV card in the IDMS. The regional POC must notify the local physical access office for each building that the card holder had access to. These offices will be instructed to revoke the local building access belonging to the cardholder until a new card can be issued.

The HSPD-12 Program Office ensures the above requirements are met prior to authorizing issuance of a new PIV card.
