FEMA – Federal Emergency Management Agency

1. BACKGROUND

The immediate response to a catastrophic disaster, act of terrorism or other emergency involves local public safety, emergency medical services, public health, and health care organizations. While most emergencies are handled locally, major incidents may require assistance from other jurisdictions, including the State and Federal governments.

The National Response Framework (NRF), within the construct of the National Incident Management System (NIMS), is an all-hazards plan that provides the structure and mechanisms for national-level policy and operational coordination for domestic incident management. The NRF identifies the Federal Emergency Management Agency (FEMA) as the lead for the Federal Government in coordinating and leading the unified Federal Response during a major/complex incident. FEMA works in partnership with other organizations that are part of the nation's emergency management system including state and local emergency management agencies, and Federal agencies, such as the U.S. Department of Health and Human Services (HHS) and the Department of Homeland Security (DHS) Office of Health Affairs (OHA)

2. PURPOSE

The purpose of this requirement is to obtain Contractor-managed multi-functional medical support resources and capabilities as needed in support of Federal assistance. The support resources shall be in the form of both staffed medical transport capabilities as well as licensed and certified medical personnel for augmentation beyond medical transportation.

3. SCOPE

FEMA has a need for medically related support services to include but not be limited to:

1) Medical Transportation

a) Advanced Life Support (ALS) or Basic Life Support (BLS) Ground Ambulances

b) Para-transit support

c) Medical Airlift

2) Medical Personnel

a) Emergency Medical Technician (EMT)

b) Paramedic

3) Field Operations Team (FOT)

4) Communication Support Team (CST)

5) Planning and Exercise Support Services

The Contractor must be able to provide all five services in the zone.

4. PERIOD OF PERFORMANCE

The period of performance is for a 12-month base period starting at contract award, with four (4) optional 12-month periods.

Task order period of performance shall be stated upon issuance.

5. PLACE OF PERFORMANCE

FEMA has divided the 48 contiguous states into four geographical zones (see Attachment 2). This contract provides the described services to the Western U.S. and/or Central U.S. (Zones 3 and 4 respectively).

Zone 3 includes the states of Washington, Oregon, California, Idaho, Nevada, Montana, Wyoming, Utah, Arizona, Colorado, and New Mexico.

Zone 4 includes the states of North Dakota, South Dakota, Nebraska, Kansas, Oklahoma, Minnesota, Iowa, Missouri, Arkansas, Wisconsin, Illinois, Michigan, Indiana, Kentucky, Tennessee, and Ohio.

The Contractor may be requested to provide support in areas outside of the awarded zone(s). Such requests shall be a best effort with prices and costs to be determined at the time of the request.

Readiness shall take place at the Contractor’s facility. Task Orders will designate the exact locations for other services to be provided.

6. DEFINITIONS

6.1. Full Zone

A “full zone requirement” shall consist of the following resources:

· 300 medically staffed ground ambulances (a combination of ALS and BLS)

· 25 medically staffed aircraft (combination of fixed- and rotary-wing)

· 3500 para-transit seats

· 150 medical support personnel (combination of EMTs and paramedics)

· 1 Operations Support Team (to be determined by scope of task)

· 1 Communications Support Team (to be determined by scope of task)

It shall be the sole responsibility of the Contractor to properly staff all deployed resources for safe, effective, and continuous operations. When resources are ordered to respond to an event, it is expected that the Contractor provide enough personnel and assets as necessary to perform the required duties 24 hours a day, seven (7) days a week during the period of performance stipulated in each task order.

6.2. Contracting Officer

The Contracting Officer (CO) has ultimate contracting authority and all contracting issues fall under his or her purview. COs are responsible for administering the contract, including the resolution of disputes and protests and terminating and/or closing out of the contract. COs are responsible for ensuring all contractors receive impartial, fair, and equitable treatment.

6.3. Contracting Officer’s Technical Representative

Contracting Officer’s Technical Representatives (COTRs) are specifically appointed by the CO and delegated limited responsibilities for inspecting and accepting work performed by the contractor. COTRs are responsible for assisting and advising the CO on technical matters relating to issuing task orders and administering the contract. COTRs are not authorized to change any terms of the contract.

6.4. Technical Monitor

The Technical Monitor (TM) assists the COTR in defining the requirement(s) of task orders issued under the contract and inspecting the subsequent work products.

6.5. Organic

In the context of this document, organic refers to assets and capabilities owned or wholly controlled by the Contractor.

6.6. National Event

A national event, as used in this document, is when federally provided medical transportation and support capabilities are needed to support activities that otherwise exceed available local and state resources.

7. PERSONNEL

7.1. Project Director

The Contractor shall provide a Project Director who shall be responsible for all Contractor work performed under this IDIQ contract and has the authority to obligate the Contractor to the terms and conditions of the contract as well as operational task orders. The Project Director is further designated as Key Personnel by the Government. The Contractor shall supply a resume of the proposed project director for evaluation by FEMA.

The Project Director shall be the primary point of contact (POC) for the CO and the COTR. There shall be at least one Alternate Project Director designated by the Contractor and communicated to FEMA in the event the primary Project Director is unavailable to perform his/her duties. The Alternate Project Director is also designated as Key Personnel.

The Project Director and/or Alternate Project Director shall be available to the CO and COTR via telephone and email on a 24 hours, seven (7) days a week basis.

Before replacing any individual designated as Key Personnel by the Government, the Contractor shall notify the COTR in advance and provide an acceptable substitute to the Government. The Government may designate additional Contractor personnel as Key Personnel in each Task Order awarded under this contract.

The Project Director shall be a senior level employee empowered to execute and direct on behalf of the contractor. The Project Director shall have no less than 10 years of direct emergency management and/or medical administration experience. In the absence of the Project Director, the Alternate Project Director shall be empowered to execute and direct on behalf of the contractor. The Alternate Project Director shall have no less than 10 years of direct emergency management and/or medical administration experience. Additionally, the Project Director and all designated alternates shall be able to read, write, speak and understand English.

7.2. Medical Liaison

The medical liaison shall coordinate between the deploying ambulances and the existing local medical direction and control system to ensure the number of assets ordered are integrated and comply with local medical directives and protocols. Medical direction and control will be provided by one or more of the following methods as determined by FEMA:

· Utilize existing local medical control through a contracted medical doctor.

· State or Regional EMS medical director(s) will perform this function.

· HHS will perform this function through one of its own physicians if required

· Contractor shall provide medical direction and control.

The Medical Liaison must be a person who maintains a current medical license as a physician in good standing. Additionally, the Medical Liaison and all designated alternates shall be able to read, write, speak and understand English.

7.3. Medical Personnel

The Contractor shall provide qualified personnel to perform all requirements specified in Task Orders awarded under this IDIQ contract. All sub-Contractors shall be with licensed and credentialed (in their State of origin) ambulance provider organizations. FEMA will assist in getting the affected state(s) to allow reciprocity for Contractor and subcontractors responding pursuant to this contract.

The functions authorized above may be performed by the certified EMT under any of the following conditions:

· While he/she is at the scene of a medical emergency where voice contact is established with a physician and under a physician’s order.
