ATTCHMENT 3: STATEMENT OF WORK

STATEMENT OF WORK (SOW)

Side Scan Ultrasonic Testing of Underwater and Freeboard Plating Services

1. GENERAL: This is a non-personal services requirement to provide Side Scan Ultrasonic Testing services at the U.S. Coast Guard Shipyard, 2401 Hawkins Point Road, Baltimore, MD. Performance shall take place on vessels in dry-dock. The Government shall not exercise any supervision or control over the Contractor employees performing the services herein. The Contractor’s employees shall be accountable solely to the Contractor who, in turn, is responsible to the Government.

1.1 DESCRIPTION OF SERVICES/INTRODUCTION: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, planning, and other items and non-personal services necessary to perform Side Scan Ultrasonic Testing services as defined in this Statement of Work (SOW) except for those items specified as Government furnished property and services. The Contractor shall perform to the standards in this contract.

1.2 BACKGROUND: It is anticipated that the U.S. Coast Guard Yard will require Side Scan Ultrasonic Testing services for upcoming vessels scheduled for Yard Availability. The vessel lengths vary among 270’ Medium Endurance Cutters, 225’ Seagoing Buoy Tenders, 210’ Medium Endurance Cutters, 175’ Coastal Buoy Tender, 140’ Ice Breakers, Eagle 327’, 110’ and 87’ Patrol Boats, 65’ Tender/Barge and 75’ Tender/Barge.

1.3 OBJECTIVES/SCOPE: The Contractor shall provide the Side Scan Ultrasonic Testing services with suitable side scan equipment (The echo to echo transducer is not authorized due to inconsistent readings and technical difficulties), to conduct a “Scanning Crawler Automated Test” of the vessel’s underwater (u/w) hull and freeboard plating for the presence of metal corrosion and deterioration. Ensure the following:

· All Scanning operations shall be completed by personnel meeting the following qualifications at a minimum: American Society for Nondestructive Testing (ASNT) Recommended Practice No. SNT-TC-1A.

· An SNT Level III Analyst shall be on site to oversee the project and evaluate all data collected.

· An SNT level II or III Analyst shall be on site to verify with a hand held gauge all areas on the hull identified as thinner than the criteria specified below.

· The above qualified personnel are on site to complete corrosion inspections as needed.

· The echo to echo transducer is not authorized due to inconsistent readings and technical difficulties.

1.3.1 SCANNING PARAMETERS: The Contractor shall use the following parameters during the course of this inspection:

1.3.1.1 FOR 87’ VESSELS:

· Plates where scan data indicating wastage greater than or equal to 60% nominal plate thickness shall be highlighted in “RED” and designated “repair areas”

· Plates where scan data indicating wastage greater than or equal to 25% to 59% of nominal plate thickness shall be highlighted in “YELLOW”

· Plates where scan data indicating wastage greater than or equal to 15% to 24% of nominal plate thickness shall be highlighted in “LIGHT GREEN”

· Plates where scan data indicating wastage greater than or equal to 10% to 14% of nominal plate thickness shall be highlighted in “DARK GREEN”

1.3.1.2 FOR ALL OTHER VESSELS

· Plates where scan data indicating wastage greater than or equal to 25% nominal plate thickness shall be highlighted in “RED” and designated “repair areas”.

· Plates where scan data indicating wastage greater than or equal to 15% to 24% of nominal plate thickness shall be highlighted in “YELLOW”

· Plates where scan data indicating wastage greater than or equal to 10% to 14% of nominal plate thickness shall be highlighted in “DARK GREEN.”

1.3.1.3 The Contractor shall ensure that automated scanning resolution is set to 0.5” x 0.5”

1.3.1.4 The Contractor shall measure the average coating thickness using 12 random readings per side of the vessel. Subtract the average coating thickness from all the measurements to approximate the actual steel thickness for each point. Include the random readings with their approximate location of the vessel and calculated average in the final report.

1.3.1.5 The Contractor shall make all hull markings with a paint pen.

1.3.1.6 Hand held Ultrasonic Thickness (UT) readings shall be taken to verify bad metal areas found with side scan equipment. All readings shall be provided to the COR and the lowest reading marked on the hull at the bad metal location.

1.3.1.7 Areas that are accessible but cannot be reached by the automated scanner shall be scanned with a hand held meter at a resolution of 2” x 2”. Any repair areas found during the hand scan shall be marked on the ship’s hull and their approximate positions recorded on the plate drawings and report summary.

1.3.2 SIDE SCAN INSPECTION AREAS:

1.3.2.1 The Contractor shall scan areas including the Port, Starboard and Transom exterior shell, but shall exclude the following.

· Areas under and within 6-inches of blocks and side blocks (areas that are accessible 6 inches from the blocks and side blocks must be scanned).

· Areas up to 6-inches around any obstacles on the hull including sea suctions, discharges, rudderposts, knuckle, bow stem, stabilizer tubes, and stern tubes as applicable.

· Any areas blocked by immovable scaffolding or shipyard activity.

1.3.3 SIDE SCAN REPORT: The Contractor shall submit to the COR a final report within 3 working days after the side scan inspection is completed. A copy of all repair areas shall be clearly marked on the provided shell plate drawing(s), to include a summary document of the same areas. Submit two hard copies of the report with two CD’s containing the electronic (raw) data.

1.3.3.1 This report shall be in written and electronic format and shall include the following:

· Inspection overview.

· Plate summary: the summary must be in table format with, at a minimum, a column for plate identification, repair identification, location of repair area, size of repair area, and brief description, i.e exterior pitting, hole interior corrosion. Data sheets for each plate with designated repair areas outlined in blue.

· Mosaic of C-Scan data overlaid on shell plate expansion U. S. Coast Guard Drawing:

· 87’ vessels: 87 WPB 111-001

· 270’ vessels: A class 901 WMEC 111-001, 901 WMCE 111-002 and 901 WMEC 111-003. B class 905 WMEC 111-001, 905 WMCE 111-002 and 905 WMEC 111-003,

· 210’ vessels: A class 618 WMEC 111-001, 618 WMEC 111-003, 618 WMEC 111-004, B class 627 WMEC 111-001, 627 WMEC 111-003, 627 WMEC 111-004.

· 140’ vessels: 140 WTGB 111-001

· 175’ vessels: 175 WLM 801-019

· 225’ Vessels: 225 WLB 111-001

· 75’ vessels: 75 WAGL 1101-001, 68 CGB 0103-8

· 65’ vessels: 65 WYL 1101-001, 70 CGB 0103-002

· C-Scan: A separate page for each plate with a C-Scan image of that plate only. The upper right or lower right of the page must contain the plate identification for easy retrieval. The header or footer must also contain the plate design weight, nominal thickness, criteria thickness for good/green, criteria thickness for caution/yellow, and criteria thickness for repair/red. All repair areas must be outlined in blue in the C-Scan image as they are marked on the hull. Each repair area must have a label corresponding to the label on the hull. All unusual areas on the C-Scan such as rough surface, stainless insert, etc. must be called out and noted in blue.

· AutoCAD drawing: The Contractor shall provide a separate AutoCAD drawing for each plate with repair areas identified. The upper right or lower right of the page must contain the plate identified for easy retrieval. All repair areas must be drawn to scale and dimensioned. Orientation of the plate to the boat must be identified. Assume the lower left corner of the plate is a right angle for drawing purposes. Use the left and bottom weld for obtaining dimensions of repair areas. Drawing tolerances can be +/- ½”.

· Digital photos of all designated repair areas. One digital photo of each plate with repair areas identified. The upper right or lower right of the page must contain the plate identification for easy retrieval.

· All electronic files shall be put on CD containing a complete copy of the report, interactive data, and digital pictures. All electronic files shall be compatible with standard U. S. workstation operating software. The interactive data file will allow you to change the thickness criteria for each color as a percentage of wastage or material thickness, the program must be able to generate a new C-scan image per the new inputted criteria. Electronic files format shall be such that the CG has the capability to change the percent of wastage (format conditioning) per plate. Electronic files shall be capable of being opened using MS Word, MS Excel, or PDF software. Digital pictures shall be recorded in JPEG format.

1.3.4 The Contractor shall conduct an out-brief with U.S. Coast Guard Yard Personnel within 24 hours of completion of tasking which will include the following:

· Inspection overview

· Explanation of findings including areas in need of repair.

1.3.5 The Contractor shall provide the requested side scan services within 48 hours of notification. This notification will be issued in the form of a task order or verbal authorization by the contracting officer. All work will be authorized by task orders, detailing the location and amount of services to be provided. The Government, at its discretion, may modify the task order to add/reduce the estimated scope of work and/or duration required.

1.3.6 The Contractor and Contractor employees are subject to and shall observe and comply with all applicable U.S. Coast Guard regulations, policies, and procedures, e.g.: fire, safety, sanitation, environmental protection, security, gratuities, flag courtesy, "off limit" areas, wearing of parts of military uniforms, personal appearance, parking, moving vehicles, use of drugs and intoxicants and possession of firearms or other lethal weapons. The Contractor shall be responsible for the selection, training, certification, assignment, supervision, management, and control of employees in the performance of all contract work. The removal from the job site of any employee shall not relieve the Contractor of the requirement to provide sufficient personnel to perform the terms and conditions of the contract.

1.4 GENERAL INFORMATION

1.4.1 QUALITY CONTROL: The Contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this SOW. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract.

1.4.1.1 The Contractor shall submit, for Contracting Officer approval, a Quality Control Plan as part of its proposal. The plan shall include, at a minimum, the following:

a. The quality control organizational structure.

b. A description of the methods of identifying deficiencies in the quality of services performed before the level of performance becomes unacceptable.

c. A description of the methods of documenting and enforcing quality control operations of Contractor, and any Subcontractor, work, including inspection and testing

1.4.2 RECOGNIZED HOLIDAYS: The Contractor may be required to work on the following federal holidays:

1.4.3 HOURS OF OPERATION: The U.S. Coast Guard Yard normal working hours are from 7:00 A.M. to 3:30 P.M., Monday thru Friday, except Federal holidays or when the Government facility is closed, due to local or national emergencies, inclement weather, administrative closings or similar Government directed facility closings. During the months of June thru August the U.S. Coast Guard Yard may switch to summer working hours which are 6:00 A.M. to 2:30 P.M Monday thru Friday.

1.4.3.1 The Contractor shall at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this SOW when the Government facility is not closed.

1.4.3.2 To accommodate Yard work the Contractor may be required to work outside of the normal working hours, nights and possibly on weekends. Advance notice will be given, as much as practical.

1.4.3.3 On days of inclement weather the Contractor shall call the U.S. Coast Guard Yard inclement weather hotline at (410) 636-7910, option #1 for that day’s status. If the U.S. Coast Guard Yard is closed the contract employees will not report for that day. If there is a delayed opening the contract employees should report at that time specified in the announcement message.

1.4.4 PLACE OF PERFORMANCE: The work to be performed under this contract will be at the U.S. Coast Guard Yard, Curtis Bay Baltimore, MD 21226.

1.4.5 SECURITY REQUIREMENTS: The U.S. Coast Guard Yard, Baltimore, MD is a controlled access area. The Contractor shall be responsible for providing company issued photo identification to all employees directed to perform work on the U.S. Coast Guard Yard. All Contractor employees are required to identify themselves with company identification on their hard hats and/or uniforms. Any Contractor employee, not following U.S. Coast Guard rules and regulations, will be removed from performance on this contract and may be barred from reentering the U.S. Coast Guard Yard.

Contractor employees shall not enter restricted areas in accordance with Yard security regulations or policies. Before Contractor employees can work at the U.S. Coast Guard Yard Facility, the Contractor shall complete a security investigation on all Contractor employees in connection to this contract. Any Contractor employee with a criminal back ground must get prior authorization before they can gain access to U.S. Coast Guard Yard Facility. Contractor shall provide for each employee to the Contracting Officer, the full name and any alias, address, city and state, and date of birth.

1.4.5.1 PHYSICAL SECURITY: The Contractor shall be responsible for safeguarding all Government equipment, information and property provided for Contractor use.

1.4.5.2 LISTING OF EMPLOYEES: The Contractor personnel shall maintain a current listing of employees. This list will include the employee’s name, social security number, and date of birth. The list shall be provided to the Contracting Officer and the COR. An updated listing shall be provided when an employee’s status or information changes.

1.4.5.3 REPORTING REQUIREMENTS: Contractor personnel shall report to an appropriate authority (Contacting Officer, COR, Ship Superintendant) any information or circumstances of which they are aware that may pose a threat to the security of U.S. Coast Guard personnel, Contractor personnel, resources, and classified or unclassified defense information. The Contractor shall not substitute employee(s) without prior notification to the Contracting Officer and COR. Contractor employees shall receive a full safety briefing by their immediate supervisor upon initial on-base assignment of all known hazards in areas that work shall be performed.

1.4.6. OCCUPATIONAL SAFETY AND HEALTH STANDARDS: The Contractor is required to follow 29 CFR 1915 (OSHA Shipyard Industry Standard), while performing work at the U.S. Coast Guard Yard. The Contractor shall provide all required personnel safety equipment for their employees, such as, but not limited to; safety helmet, safety glasses with side shields. All safety equipment must meet OSHA standards. Failure of the Contractor employees to comply with the above requirements could result in a shutdown of operations.

1.4.6.1 The U.S. Coast Guard Yard has 24 hour first aid, spill response and fire protection available. If necessary, the Contractor may call 410-636-3993 from non-Yard phones and extension 3000 from Yard phones for emergency situations. The U.S. Coast Guard Yard will provide emergency first aid only. Injuries must be reported immediately and followed up with a written report to the Contracting Officer Representative (COR).

1.4.7 International Organization for Standardization (ISO) QUALIFICATIONS: The Contractor shall comply with all the U.S. Coast Guard Yard ISO 9001 Quality Management and ISO 14001 Environmental Management Systems certifications. The Contractor may be periodically audited to ensure conformity to these certifications and standards. In the event the Contractor creates an unsafe work environment and will cause yard work operations to cease, the Contractor shall be held responsible and damages shall be assessed for the length of the work delay.

1.4.8 POST AWARD CONFERENCE/PERIODIC PROGRESS METTINGS: The Contractor shall agree to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation (FAR) Subpart 42.5. The Contracting Officer, COR, and other Government personnel, as appropriate, will meet before any work commences and may meet periodically with the Contractor to review the Contractor's performance and schedule. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the Government.

1.4.9. CONTRACTING OFFICER’S REPRESENTATIVE (COR): The COR will be identified by separate letter. The COR will monitor all technical aspects of the contract and assist in contract administration. The COR is authorized to perform only those functions designated in the letter, issued by the Contracting Officer. A letter of designation, issued to the COR (a copy of which will be sent to the Contractor), states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in performance dates. The COR is not authorized to change any of the terms and/or conditions of the contract. All contract changes that impact price or cost, quality of work and the performance schedule may only be approved or made by the Contracting Officer

1.4.10. KEY PERSONNEL: The Contractor shall provide a Supervisor who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the Contractor when the Supervisor is absent shall be designated in writing to the Contracting Officer. The Supervisor or alternate shall have full authority to act for the Contractor on all contract matters relating to daily operation of this contract. The Supervisor or alternate shall be available between 7:00 a.m. to 4:30 p.m., Monday thru Friday except when the Government facility is closed for administrative reasons.

1.4.10.1 ANALYST:

1.4.10.1.1 The Analyst must have at a minimum two (2) years of documented experience supervising side scan operations, meet the certification requirements from ASNT recommend practices number SNT-TC-1A for a SNT level III Analyst. The ability and experience of the analyst shall be provided to the Government with the proposal in the form of a resume detailing qualifications such as experience, training and certifications. A copy of all certifications for the analyst shall be provided to the Government with the proposal.

1.4.11 SCANNING PERSONNEL

1.4.11.1 Scanning Personnel: Scanning personnel shall have a minimum two (2) years documented experience providing scanning operations, meet the certification requirements from ASNT recommend practices number SNT-TC-1A. The ability and experience of the scanning personnel shall be provided to the Government with the proposal in the form of a resume detailing qualifications such as experience, training and certifications. A copy of all certifications for the scanning personnel shall be provided to the Government with the proposal.

1.4.12 COORDINATION WITH COR: The Contractor shall coordinate all work requirements, stated herein, with the Contracting Officer and the COR, the COR will communicate with the Supervisor to ensure adherence with the Task Order and SOW. There may be occasions in which the U.S. Coast Guard Yard will request the Contractor to cease work or move to a different location on the vessel due to U.S. Coast Guard Yard production requirements.

1.4.13 INTERFERENCES: Work sites may contain such items as tools, hoses, debris, and other interferences from U.S. Coast Guard Yard work operations. The Government shall attempt to remove and/or minimize such interferences to the extent that it is practical. In the event such interferences are not removed, however, the Contractor shall notify the COR immediately to devise an alternate work plan to allow work to commence. The presence of interferences and the COR’s inability to totally mitigate the impact on performance will not relieve the Contractor of responsibility for performance of services required.

1.4.14 DAMAGE TO GOVERNMENT PROPERTY: The Contractor shall be held liable for any damage to Government property caused by the Contractor. The Contractor shall provide protective coverings on all vulnerable equipment and surfaces. It shall be the responsibility of the Contractor to ensure that the covering is adequate. Any concerns as to the adequacy of the coverings must be brought to the attention of the COR. The Contractor shall inspect the area before work commences and may request a technical inspection from the COR, however such an inspection does not relieve the Contractor from responsibility for damage.

PART 2

DEFINITIONS & ACRONYMS

2. DEFINITIONS AND ACRONYMS:

2.1. CONTRACT DEFINITIONS:

2.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the Government. The term used in this contract refers to the prime.

2.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the Government. Note: The only individual who can legally bind the Government.

2.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

2.1.4. KEY PERSONNEL. Contractor personnel whose qualifications are evaluated in the source selection process and whose services the Government can require as part of the performance of the contract in accordance with the Key Personnel provisions in the SOW. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal. Substitution of all Key Personnel requires approval by the Contracting Officer.

2.1.5. QUALITY ASSURANCE. The Government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

2.1.6. QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract and commercial quality requirements.

2.1.7. SUBCONTRACTOR. One that enters into a contract with a prime Contractor. The Government does not have privity of contract with the subcontractor.

3. GOVERNMENT FURNISHED ITEMS AND SERVICES:

3.1. UTILITIES: The Government will provide water, compressed air, and electricity for the Contractor as defined herein. The Contractor shall instruct employees in utilities conservation practices. The Contractor shall be responsible for operating in such a manner that precludes the waste of utilities, for example turning off the water faucets or valves after using the required amount to accomplish assigned work.

3.1.1. WATER: The Government will provide a source of water for Contractor machinery. The Contractor shall be responsible for ensuring that their water hookup connections are compatible with the Yard’s water sources.

3.1.3. ELECTRICITY: The Government will provide up to 200 amps and up to 480 volts of electrical services.

3.1.4 EQUIPMENT: The Government will only provide manlifts or staging and scaffolding in those areas that are determined not to be accessible by manlift. The Contractor shall provide proof of manlift training for employees required to operate manlifts.

3.2 The Civilian Cafeteria in building 68 may be used by all Contractor employees. Contractor employees are not authorized to attend picnics, cookouts or any other type of workday ceremonies sponsored by the government.

3.3 Contractor owned vehicles shall only be allowed on the CG YARD facility to facilitate the flow of materials, supplies, or services to and from the job site. Routine operation or parking of Contractor or privately owned and/or operated motor vehicles is not authorized within the CG YARD, without exception.

CONTRACTOR FURNISHED ITEMS AND SERVICES

4. CONTRACTOR FURNISHED ITEMS, SERVICES AND RESPONSIBILITIES:

4.1 GENERAL: The Contractor shall furnish all materials and equipment to accomplish all work, other than those explicitly address in Section 3 of this SOW. This includes ladders and/or stools to reach areas above their employee’s head.

4.1.1 The Contractor shall report to the designated work site on the date specified on the written notification by the Contracting Officer in the form of a signed task order. No other individual has the authority to direct the Contractor to begin work, other than a warranted Contracting Officer. Pending U.S Coast Guard Yard production schedules, services may be required on an intermittent basis within the time period specified.

4.2 Equipment: The Contractor shall throughout the performance of this contract be in possession of fully functioning equipment and tools to complete assigned work. The following tools and equipment are required:

a. Automated Scanning Crawler (The echo to echo transducer is not authorized due to inconsistent readings and technical difficulties)

b. Handheld UT meter

4.2.1 The above mentioned equipment may be owned or leased by the Contractor and the demonstrated ability to own or lease the equipment must be indicated in the proposal.

4.3 Materials/Safety Equipment: The Contractor shall, throughout the performance of this contract, furnish all training, material, equipment, and supplies necessary for contract performance, not identified as Government provided material. The Contractor shall provide all standard Personal Protection Equipment (PPE) in accordance with Occupational Safety and Health Administration (OSHA) Standard. The following list provides examples of typical equipment used: Hard Hat, appropriate eye protection, and all safety equipment, steel-toed shoes, ear plugs, and flash lights.

4.4 HARD HATS: Shall have the employee’s name, as it appears on identification card, designated on the front and the Company’s name on both sides. The lettering shall be standard commercial lettering (not hand printed), no less than ½ inch in height for the Company’s name and no less than 3 /8” in height for employee’s name. Lettering color is to be black. Other company officials visiting the work site shall wear a hard hat that is marked with the official’s name on the front and the company’s name on both sides of the hat.
