| Reference | Page | Question | Government Response | |
| 1 | Current contract | What are the bill rates on the current contract? | USCG will not release the incumbent's pricing | |
| 2 | General and SF1449 and Solicitation | 36 of 62 | The solicitation was released at 9:44 AM on Saturday 29 April 2017. The SF 1449 states a due date of Monday 22 May at 9:00 AM. Hardcopy submittals are required. Delivery via courier or hand-delivery is not permitted. |
This schedule requires offers to develop, produce, and ship their proposals no later than close of business on Thursday 18 May 2017 to ensure delivery on Friday 19 May 2017 because normal delivery cannot be achieved before 9:00 AM. The schedule leaves only 13 business days to develop, proof, finalize, and ship proposals to achieve timely delivery.
We respectfully request the Government revise the due date to no earlier than 1 Jun 2017 to allow a reasonable time for proposal development, production, shipment, and timely delivery.
| Additionally, we respectfully request the time be revised to 2:00 PM to allow for timely delivery on the due date via FedX or UPS. | See Amendment 00002 | ||||
| 3 | Questions Due Date | 40 | According to the solcitiation "All questions shall be submitted by email to Deborah.Culver@uscg.mil and Dorothy.J.Pelis@uscg.mil no later than 1400 Eastern Time not later than March 24, 2017." Since this date passed, please clarify actual due date for questions. | See Amendment 00001 | |
| 4 | SF 1449; FAR Provision 52.212-1, “Instructions to Offerors – Commercial Items” | 36 of 62 | The due date/time (0900 on a Monday morning) combined with the requirement that hand delivery is prohibited effectively reduces the already compressed response period by an additional four days. Time is already at a premium to respond effectively to this Solicitation. We therefore request a delay of three days or more to be able to take advantage of the preceding weekend. | See Question #1 | |

| 26 | Solicitation | 37 and 42 | Paragraph 2 provides the option for Offerors to propose on one region, two regions, or all three regions. However, Factor 3 on page 42 requires Attachments 2 and 9 to have all pricing completed for each item or the proposal will be considered non-compliant. | 27 | Solicitation | 37 | Paragraph 2 states, "the Government reserves the right to make one award for all three regions…or separate awards for each region in any combination." | 29 | Solicitation | 39 | Volume II: Past Performance defines like or similar contracts as multidiscipline healthcare services performed at multiple locations across the U.S. of similar scope, magnitude and complexity. As most medical services contracts are issued as IDIQs with various task orders, how should the offeror demonstrate the relevancy if there are multiple points of contact for different task orders under the contract preventing the Offeror from obtaining a Past Performance Questionnaire (Attachment B) representing the entire contract scope. | IDIQ contract should indicate multiple locations if that is a requirement | |
| 30 | Solicitation | 40 | Price and any discount terms states, "The Offeror shall submit the labor rate breakdown electronically in MS Excel spreadsheet format. The format shall be PC compatible and submitted via e-mail to the Contract Specialist Deborah.Culver@uscg.mil and the Contracting Officer, Dorothy.J.Pelis@uscg.mil." Please confirm if Offerors should include the labor rate breakdown as part of the proposal submission hard copy, CD, or both. If labor rate breakdown is not to be submitted with the hard copies and CD, please specify a deadline to e-mail the labor rate breakdown to the e-mail addresses noted. | Both | |
| 31 | Solicitation | 38 of 62 | The RFP requires that Offerors 'include a complete manning chart verifying the number and categories of personnel provided to each clinic. The manning chart shall show the distribution of hours (full time or part time) to provide the required full time equivalent totals and percentages specified by the Government.' |
| Attachment 9 Manning Levels provides the exact distribution of hours per labor category for each clinic. Is it the government's intent for the Offeror to include an exact copy of Attachment 9 in Subfactor 3? Is the government looking for anything additional other than what has already been provided in Attachment 9? | Attachment 9 | ||||
| 32 | Solicitation | 36 | The solicitation indicates that proposals shall be limited to 100 pages all inclusive, without hyperlinks. Please identify if anything is excluded from this count, such as FAR clauses and table of contents. | See Government Response to Questions #10 | |
| 33 | Solicitation | 38 | In reference to Volume II - Past Performance, would the government like a chart of what was submitted with past performace? In our experience this has helped clarify what has been distributed to our customers. | Offeror determination | |
| 34 | Solicitation | 4 | In Part 1, paragraph 3.b.i states that the contractor shall not escalate labor rates for SCA positions in the option periods. Does this imply that we cannot escalate any WD position rates to meet projected market demand increases? | Correct as stated | |
| 35 | Solicitation | 38 | We intend to submit a proposal as an SBA approved Mentor Protégé JV. Are past performance for each member of the JV and subcontractors acceptable to the governement? | 36 | Solicitation | 32 | Are there any formatting requirements requested in the proposal? | See A00002 Schedule of Changes Item #3 | |
| 37 | Solicitation | 37, 38 | The RFP makes references to Attachment C in " FAR PROVISION 52.212-1 -- INSTRUCTIONS TO OFFERORS" in Volume I-Technical &Management-Subfactor 3 as well as in Volume III-Cost/Price - Completed Service Breakdown/Rate Form. There is not an Attachment C included in the solicitation, can the government confirm this should be Attachment 9? | Confirm Attachment 9 | |
| 38 | Solicitation | 37, 38 | The RFP makes references to Attachment C in " FAR PROVISION 52.212-1 -- INSTRUCTIONS TO OFFERORS" in Volume I-Technical &Management-Subfactor 3 as well as in Volume III-Cost/Price - Completed Service Breakdown/Rate Form. Should this Attachment be included in both Volume I - Technical & Management as well as Volume III-Cost/Price? | No, only one need be provided | |
| 39 | Solicitation | 2 | The second page of the RFP indicates that the offeror is to submit completed contract Administration Information, however, there is no other reference as to what is required to be provided. Can the government elobrate on the requirement? | See Government Reponse to Question 11 | |
| 40 | Solicitation | 41 | Factor 2 - Past Performance: The Past Performance requirement for offeror to have 3 similar contracts to be relevant limits the competition of qualify small buisness within the $7M and $10M size standards. Will the Government amend the sentence to indicate similar contracts refers to Multidiscipline Healthcare services performed at multiple locations across the US of similar scope and complexity? | No - this requirement is for Multidiscipline Healthcare services at various locations throughout the US and Puerto Rico so similar would refer to the requirement | |
| 41 | Solicitation | 4 | Can the government provide additional clarification regarding the meaning of "Discounted invoices" reference in page 4 of 62. Is this a deduction from hourly rate invoiced by the Contractor? How would the government calcculate this discounted invoices? | See Government Reponse to Questions 13 | |

| 44 | Solicitation | 25 | Safeguarding of sensitive information: Will the government provide HCPs training concerning on HIPAA during orientation and annually? | Yes | |
| 45 | PWS | 1 | The PWS provides the general hours as "between the hours of 6:00 A.M and 4:30 P.M, Monday through Friday (excluding Federal Holidays), consisting of an eight (8) hour workday with an uncompensated lunch", however certain positions will only be providing services for a couple of day, per week. Are the Manning Charts supposed to show the breakdown of personnel per day, per week or per month? For the position that requies only 12 or 24 hours per week, on which days and hours will the sites will be requiing services? | Attachment 9 shows both the weekly and yearly hours for part-time contract employees. Days/hours will be provided at time of award | |
| 46 | Solicitation | 40, Volume III #5 | Indicate: proposals envisioning compensation levels lower than those of predecessor contractors for the same work will be evaluated on the basis of maintaining program continuity, uninterrupted high-quality work, and availability of required competent professional service employees. This create an uncompetitive pricing advantage to the incumbent by basing the pricing evaluation on the incumbent's pricing. In order to establish a competitive level plain field, will the governement provide current compensation levels for required positions? | No, that is the nature of all competive solicitations | |
| 47 | Solicitation | 36 of 62 | Please confirm whether proposals are limited to 100 pages total (for all volumes) or is Volume I page-limited to 100 pages. | See Government Response to Question #25 | |
| 48 | Solicitation | 37 of 62 | In reference to Subfactor 3, paragraph 2, please clarify what deliverable is required to satisfy the manning chart requirement given that the distribution of hours is defined in Attachment C/Attachment 9. If the hours are provided by the Government, what specifically is needed to satisfy this requirement? | Completion of Attachment 9 | |

| 51 | (a) FAR Provision 52.212-1, “Instructions to Offerors – Commercial Items” and (b) FAR Provision 52.212-2 “Evaluation – Commercial Items” | (a) 38 of 62; (b) 41 of 62 | It is unclear whether a Quality Control Plan is required to be submitted with the proposal. Subfactor 2 – Management Approach describes what must be delineated in the QCP but does not specify its submittal. The evaluation factors mention the Quality Control Program within the Offeror’s proposed management approach discussion but not a Plan per se. Please clarify requirement. | Quality Control Plan is not required to be submitted with proposal. Offeror's will provide a general overview of the its Quality Control Plan on order for the Government to determine the Offeror's understanding of the requirement. | |
| 52 | 52.212-2 -- Evaluation -- Commercial Items (Oct 2014); Factor 1 – Technical and Management Approach, Subfactor 1 | 41 of 62 and page 22 of 62 | The Section states “The Offeror’s proposed technical approach to provide multidiscipline healthcare services, at a minimum, detailed staffing procedures/hiring process, pool of candidates, pre-qualification of candidates, and malpractice insurance.” |
Malpractice insurance is not required on personal service contracts. Should this reference HSAR Clause 3052.228-70 -- Insurance (DEC 2003) (i.e., insurance with respect to performance under the contract)?
| If not, is the Government requiring malpractice insurance over and above that provided by the Government on personal service contracts? | See Government Response to Question #123 | |||
| 53 | FAR PROVISION 52.212-1 -- INSTRUCTIONS TO OFFERORS | Page 37 of 62 | The first paragraph states: "Proposals shall be limited to 100 pages all inclusive with page numbers in sequential order. The use of hyperlinks in proposals is prohibited." |
This appears to require offerors to page number Volumes I, II, III in sequential order (i.e., Volume II numbering begins where Volume I ends and Volume III numbering begins where Volume II ends).
Is this understanding correct?
If "No" is it acceptable to page number each volume separately?
| Additionally, we respectfully request Volume III, Cost/Price be excluded from the page limit. | See Government Response to Questions #25 | ||||
| 54 | FAR PROVISION 52.212-1 -- INSTRUCTIONS TO OFFERORS -- COMMERCIAL ITEMS (Jan 2017), included in full text, tailoring is included as specified pursuant to FAR 12.302. Volume II: Past Performance Offerors shall submit at least three (3) relevant past performance references for contracts with the private industry or government instrumentalities (Federal, state, or local) utilizing the Past Performance Questionnaire (Attachment B). | 39 | Please provide Attachment B. | Attachment B is Attachment 10 | |
| 55 | FAR PROVISION 52.212-1 -- INSTRUCTIONS TO OFFERORS -- COMMERCIAL ITEMS (Jan 2017), included in full text, tailoring is included as specified pursuant to FAR 12.302. Proposals shall be limited to 100 pages all-inclusive with page numbers in sequential order. | 37 of 62 | We want to be sure we comply with this requirement. As written, it seems that the 100-page limit is all-inclusive for all volumes (potentially including Cost/Price) including all covers, Tables of Contents, etc.? Please reaffirm/clarify. | See Government Response to Question #25 | |
| 56 | FAR52.212-1 Instructions to Offerors | 37 | The instructions indicate that proposals are limited to 100 pages all inclusive, with page numbers in sequential order. Is the 100 page limit for the technical proposal only, as the instructions for the Cost/Price volume indicates there is no page limit. | See Government Response to Question #25 | |
| 57 | FAR52.212-1 Instructions to Offerors | 37 | The instructions indicate that proposals are limited to 100 pages all inclusive, with page numbers in sequential order. Is the 100 page limit for the technical proposal only, as the instructions for the Cost/Price volume indicates there is no page limit. | See Government Response to Question #25 | |
| 58 | FAR52.212-1 Instructions to Offerors, Vol I, Subfactor 1 | 37 | Since this is a personal-services contract so malpractice insurance is not required, what insurance information needs to be included in Subfactor 1 Technical Approach? Are you requesting copies of insurance certificates for the insurance coverages listed in HSAR Clause 3052-228-70? | Yes | |
| 59 | FAR52.212-1 Instructions to Offerors, Vol I, Subfactor 3 | 38 | Please clarify what is required for the manning chart. Are the offerors expected to copy the Labor Categories and Productive Hours and include them in Vol I? If no, what is expected? | See Government Response to Question #48 | |
| 60 | FAR52.212-1 Instructions to Offerors, Vol II | 39 | The instructions for Volume II Past Performance indicate that the PP Questionnaire should be utilized for this section. Since the Questionnaires will be sent directly by our references to the Government, what information should be provided in Volume II of the proposal? | Offeror determination | |
| 61 | FAR52.212-1 Instructions to Offerors, Volume II Past Performance | 39 | Can a CPARS be substituted for a Past Performance Questionnaire? | See Government response to Question #50 | |
| 62 | Instructions to Offeror | May an offeror add its required breakdown of the fully-burdened firm-fixed unit prices proposed to Attachment 2, in subsequent worksheets, or does the supporting data need to be provided in a separate MS Excel spreadsheet? | Complete Attachment 2 and Attachment 9. See Manning Levels Attachment 9 | ||
| 63 | Instructions to Offerors | 37 | Please confirm that, if an offeror is proposing on all three regions, the offeror is still limited to only one submission of VOL II (i.e. the PPISs returned will be evaluated by the Government with respect to all regions on which the offeror is proposing). | Only one submission of Volume II per proposal | |
| 64 | Instructions to Offerors | 37 | If an offeror is proposing on all three regions, would the Government like it to submit three separate VOL I Technical proposals or one? We do not see the need to submit more than one. | See Government Response to Question #63 | |
| 65 | Instructions to Offerors | 37 and 39 | Page Limits. Would the government clarify the page limitations for the volumes of this proposal and confirm that Vol I Technical & Management and Vol II Past Performance have a combined limit of 100 pages and Vol III Cost/Price has an unrelated unlimited page count? REFERENCES: Sec. L, page 37: “Proposals shall be limited to 100 pages all-inclusive with page numbers in sequential order.' and Sec. L, page 39: “Cost/Price proposal is not page limited” | See Government response to Question #25 | |
| 66 | Instructions to Offerors | 36 and 38 | Page Count. Please confirm that Volume 3 -Cost/Price is not page limited and is excluded from the statement on page 36 that “Proposals shall be limited to 100 pages.” | See Government response to Question #25 | |
| 67 | Instructions to Offerors | 38 | Past Performance. Is there a maximum number of Past Performances? | No | |
| 68 | Instructions to Offerors | 38 | Past Performance. Of the 3 required minimum past performances, what number must come from the Prime? | Offeror determination | |
| 69 | Instructions to Offerors | 38 and 41 | Attachment. Is the document referenced as “Attachment C – Manning Levels Service Breakdown Rates” the same document on FBO titled “Attachment 9 – Manning Levels Service Breakdown Rates?” Can we assume that the Attachment 9 referenced on page 41 is the same document referred to as Attachment C on page 38? | Yes | |
| 70 | Instructions to Offerors | SF 33 | Extension. Can the due date be extended by a minimum of 14 days? We have outstanding questions related to past performance that will affect which PPQs we submit. Because of the delay between solicitation release date and answers to questions, we are concerned this won’t allow enough time for our POCs to complete the PPQs on their busy schedules. | See Amendment 00002 | |
| 71 | Instructions to Offerors | 38-39 | Past Performance. Past Performance information is asked for in two volumes of this proposal and the criterion for eligibility is different in each. In Vol I, the criterion is work performed within five (5) years. In Vol II, it is work performed within three (3) years. Would the government clarify and consider making the eligibility criteria for both volumes consistent: past performance/corporate experience completed within the last five years? REFERENCES: Sec L, page 38 (Vol I Technical & Management) “The offeror should address the following: history, organization, qualifications, and work experience all within the last five (5) years as they relate to the requirements of the PWS. Sec. L, page 39 (Vol II Past Performance) “The Past performance is for services similar, in scope, magnitude, size and complexity to the services detailed in the solicitation and must have been performed within the past three years.” | Within three (3) years | |
| 72 | Instructions to Offerors | Escalation for Health and Welfare. The solicitation indicates that we are not allowed to escalate the government’s labor rates for SCA categories. Are offerors allowed to increase health and welfare because they do not increase annually on wage determination? | For solicitation evaluation purposes there is not escalation but FAR 52.222-43 provides the authority to adjust the contract price for SCA-covered labor on the Annual anniversary date/option exercise | ||
| 73 | Instructions to Offerors | 36 | Credentialing Timeline and Pool. Will the government allow pre-credentialing and security clearance for extra providers for pool? This will contribute to filling vacancies quickly and help in avoiding invoice discrepancies. | No | |
| 74 | Instructions to Offerors | Information Disclosure. The government is requesting a full indirect price build up. Would the government confirm that confirm this information will NOT be disclosed in response to FOIA requests without the offeror’s written consent, so long as this information is marked “proprietary”? | Offeror's pricing is not released in FOIA without Offeror's consent | ||
| 75 | Instructions to Offerors | Escalation for SCA Bare Labor Rates. The solicitation indicates that we are not allowed to escalate bare labor rates for SCA categories. Are offerors allowed to escalate bare labor rates in accordance with the FAR with the understanding that the contractor would only be entitled to a labor rate adjustment if the new rate in the WD exceeded the bid to pay bare rate of the contractor, otherwise the contractor would only be entitled to a health and welfare adjustment if there is an annual change? | See Government Response to Question #72 | ||
| 76 | Solicitation Section FAR PROVISION 52.212-1 -- INSTRUCTIONS TO OFFERORS, Volume II Past Performance, and Past Performance Information Sheet and Amendment 1 Delivery Instructions | 39 of 62 and Past Performance Information Sheet and Amendment 1, Page1 | The Past Performance Information Sheet instructs offeror customers to submit the completed sheet directly to the Government. (The customer POC shall submit the entire completed Past Performance Information Sheet (Sections I and II) directly to the Contracting Officer via email to Deborah.Culver@uscg.mil no later than the closing date and time of the solicitation.) The delivery instructions state: “Offerors shall provide seven (7) each hard copies of Volume 1 and one (1) each copy of Volume II and Volume III and one CD of each volume with Excel spreadsheets unlocked to the following address (no couriers or hand deliveries will be accepted)." | ||
| Question: What information is to be included in the submitted Volume II and on the Volume II CD? | Offeror determination | ||||
| 77 | Solicitation, FAR PROVISION 52.212-1 -- INSTRUCTIONS TO OFFERORS, Subfactor 2, Management Approach (4th paragraph) | 38 of 62 | The 2nd sentence states: The QCP shall include timely and effective corrective action for all deficiencies identified by the Government addressing the areas identified in the PWS; shall implement procedures to identify, prevent, and ensure non-recurrence of defective services; shall address inspection procedures; shall provide an action plan for correction of discrepancies; shall provide a strategy for retaining qualified personnel; and, shall explain management’s role in its commitment to quality performance. It appears “retention” is covered in Subfactor 3 – Manning and Key Personnel. Is retention to be addressed in both Subfactors 2 and 3? | See Government Response to Question #51 | |
| 78 | HSCG23-17-R-P1C000 Solicitation; FAR Provision 52.212-1 | 39 | The instructions for Volume III is requesting offerors to address FAR 52.222-46 (Evaluation of Compensation for Professional Employees). Do offerors need to provide a separate compensation plan aside from what is required in the Completed Service Breakdown/Rate form? | Offerors need to follow the requirement in FAR 52.222-46 | |
| 79 | HSCG23-17-R-P1C000 Solicitation; FAR Provision 52.212-1 | 38 | The Government has requested that offerors use the Manning Levels/Service Breakdown Rates Attachment to provide the required manning information to address Subfactor 3. Is this to show how many individuals will be staffing each position or is it the Government's intent that this section will be evaluated using the completed Attachment? | Completed Attachment 9 | |
| 80 | HSCG23-17-R-P1C000 Solicitation; FAR Provision 52.212-1 | 38 | Subfactor 2 of Volume I, requests that offerors provide a demonstration of "similar or directly related corporate work experience of similar scope, magnitude and complexity to the PWS" and goes on to state that the experience will be demonstrated for the "prime contractor". If offerors are proposing from a SBA-approved Joint Venture, can experience from either member of the Joint Venture be considered to satisfy the "prime contractor" requirement? | JV offeror does not have to submit past performance for the JV (although they can if they have it but not all JV's have it) - and can submit past performance for the individual participants' prior contracts as applicable (i.e., the same type work that they are performing under the JV) | |

Please confirm that a finding of unacceptable refers to ratings received on the past performance from the evaluating POC and not to any submitted contracts that the Government deems to be not relevant. | Unacceptable refers to ratings received on the past performance from the evaluating POC

We would recommend the Government revise the limitation to be no more than 35 or, at most, 50 pages, excluding the required resumes (which should not count against the volume’s page limit). Please confirm if you are requiring this information to be listed on the CV/resume or can it be provided as part of the security documents? | The CV/resume can be password protected and sent that way to meet security requirements

| 105 | 5.2.9 – Attachment 1 | 12 | It states the USCG will approve or disapprove each candidate within 14 days after submission of CV/resume package. Is it the intent of the government for a candidate to complete the entire credentialing, prime source verification, security, and HR processes before the USCG approves them to move forward with the hiring process? | No, candidate packages as submitted by the contractor are reviewed and deemed acceptable. At that point selected candidates complete the security clearance processing and credentialing process. | |
| 106 | 5.2.9 – Attachment 1 | 12 | It states the candidate is to start work 16 calendar days after government approval. We typically do not recommend candidates leaving their current positions until they know for certain they will be approved. Would the USCG consider moving that to 60 days prior to starting since the candidate will only be given 16 days notice of approval? | The physical examination must still be completed at the earliest possible date, but not beyond 30 days after the initial date of employment." So within 30 days of employment is acceptable. They should at least be able to schedule the exam during the period between notification and hire. | |
| 110 | 7.4 Health Requirements – Attachment 1 | 19 | 7.4.1a states the physical exam is performed no later than 5 days prior to commencement of work. In the previous section it states 30 days.
