“late” and may not be evaluated by the Government.

L.2 Content of Proposal:

L.2.1 Format and Instructions for Proposal Preparation: The following information shall be included with the submission of the Contractor’s Proposal:

L.2.1.1 When printed, the specified format for the narrative Proposal is as follows:

a) Pages are to be standard 8 1/2" by 11" sheet.

b) Use 12 point font, Times New Roman. The font size for tables and figures can be 8 or 10 point.

Printable graphics can be imbedded inside text files. The size/family for graphics can also be 8 or 10 point.

c) Single spacing is required.

d) Margins shall be one inch at the top, bottom, and sides, and the pages may not be in reduced form.

Headers and footers are allowed in the margins.

e) Each used side of the page will count toward the page limitations set forth below.

f) Multiple pages or foldouts are limited to 11" by 17" size paper and should be used for diagraphs or illustrations; these will individually count as one page towards the page limit. Narrative should be mailto:Kathy.L.Storaker.civ@mail.mil included in the 8 1/2" x 11" sheets.

L.2.1.2 The Contractor’s Proposal shall be prepared in accordance with these instructions. A cover letter shall be used to transmit the Proposal and is considered a part of the Proposal. The cover letter will be used to transmit any information the Contractor wishes to bring to the attention of the CO, including any assumptions and exceptions taken concerning the RFP requirements. The cover letter shall include the following information at a minimum:

a) Date submitted

b) Contractor’s name (including Cage Code, DUNS Number, and Tax Payer ID)

c) Contractor Point of Contact, contact information

d) Contractor’s contact information for those who can bind the Contractor

e) All offered Subcontractor(s) (as applicable) (including Cage Code, DUNS Number, and Tax Payer ID)

f) Reference to any unique terms or conditions

g) Contract Acceptance Period. The Contractor shall specify a period of not less than 60 working days from date of closing. This date may be extended by mutual agreement of the Contractor and the

Government.

L.2.2 Proposal Requirements and Evaluation Factors: The Contractor's response shall provide sufficient detailed information to enable an evaluation based on the following Factors.

L.2.2.1 Factor 1 – Past Performance

Contractors shall provide a minimum of three, but no more than five, past performance references with in the page limits of L.2.1. for any past or on-going work in Government and non-Government contracts for Financial

Management Support Services in accordance with the below instructions. Contractors shall also submit a list of

Government contracts (prime and major subcontracts (in performance or awarded) during the past three (3) years, which are relevant to the efforts required by this solicitation (not included in page count). Data concerning the prime

Contractor shall be provided first, followed by each proposed major subcontractor, in alphabetical order. A major subcontractor is defined as one whose subcontract is for more than 25% of the total proposed price.

L.2.2.1.1 This shall be organized into the following sections:

L.2.2.1.1.1 Contract Description: This section shall include the following information in the following format:

a) Contractor/Subcontractor’s Point of Contact (POC) name, current address, e-mail address, and telephone, place of performance, CAGE Code and DUNS Number. If the work was performed as a subcontractor, also provide the name of the prime Contractor and POC within the prime Contractor organization (name, and current address, e-mail address, and telephone and fax numbers).

b) Government contracting activity and current address Procuring CO’s name, email address, and telephone and fax numbers.

c) Government’s COR and current e-mail address, telephone and fax numbers.

d) Government contract administration activity and the Administrative CO’s name, and current e-mail address, telephone and fax numbers.

e) Contract Number and, in the case of Indefinite Delivery type contracts, GSA contracts, and Blanket

Purchase Agreements, include Delivery Order Numbers also.

f) Contract Type (specific type such as Fixed Price, Cost Reimbursement, Time & Material, etc. In the case of Indefinite Delivery contracts, indicate specific type (Requirements, Definite Quantity, and

Indefinite Quantity) and secondary contract type (FFP, CR, T&M, etc.).

g) Awarded price/cost

h) Final or projected final price/cost

i) Original delivery schedule, including dates of start and completion of work.

j) Final or projected final, delivery schedule, including dates of start and completion work.

L.2.2.1.1.2 Past Performance: Contractors shall provide a specific narrative explanation of each contract/experience listed in the Contract Description section above, describing the objectives achieved and detailing how the effort is relevant to the requirements of this solicitation.

For any contracts that did not/do not meet original schedule or technical capability performance requirements, provide a brief explanation of the reason(s) for the shortcomings and any corrective action(s) taken to avoid recurrence. The

Contractor shall list each time the delivery schedule was revised and provide an explanation as to why the revision was necessary. All Requests for Deviation and Requests for Waiver shall be addressed with respect to causes and corrective actions. The Contractor shall also provide a copy of any Cure Notices or Show Cause Letters received on each contract listed and a description of any corrective action implemented by the Contractor or proposed subcontractor. The Contractor shall indicate if any of the contracts listed were terminated and the type and reasons for the termination.

L.2.2.1.1.3 Subcontracts: Contractors shall provide an outline of how the effort required by the solicitation will be assigned for performance within the Contractor’s corporate entity and among the proposed subcontractors. The information provided for the prime Contractor and each proposed major sub must include the entire company name, company address, CAGE code, DUNS Number and type of work to be performed by citing the applicable Government

PWS subparagraph number. This includes all subcontractors whose subcontract is for more than 25% of the total proposed price.

L.2.2.1.1.4 New Corporate Entities: New corporate entities may submit data on prior contracts involving its officers and employees. However, in addition to the other requirements in this section, the Contractor shall discuss in detail the role performed by such persons in the prior contracts cited. Information should be included in the files described in the sections above.

L.2.2.1.1.5 Past Performance Assessment Questionnaire: The Past Performance Questionnaire (PPQ) included in

Section J of the solicitation is provided for the Contractor or its team members to submit to their client(s) for each project the Contractor includes in its proposal for Past Performance/Experience. Ensure correct phone numbers and email addresses are provided for the client point of contact. Completed Past Performance Questionnaires should be e-mailed directly from the client point of contact to Kathy L. Storaker, email Kathy.L.Storaker.civ@mail.mil on or prior to the closing date of this solicitation. PPQs are exempt from the Past Performance page count limitation. The

Government is not precluded from utilizing previously submitted PPQ information in the past performance evaluation.

Also include in this section the following information: performance recognition documents received within the last three (3) years such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition.

While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Contractor.

L.2.2.2 Factor 2 – Technical Capability

Contractors shall demonstrate an understanding of the technical requirements and risk in implementing those requirements. This will include the Contractor’s plans to meet all technical requirements, and the thoroughness in which the proposal addresses each technical requirement. The proposal shall address the “Technical Capability” and the “Key Personnel and Non-Key Personnel.” This shall cover all contract years. It is imperative that you fully describe how you will meet the requirements. Do not simply copy and paste, rephrase or restate PWS language.

L.2.2.2.1 Shall discuss the following:

L.2.2.2.1.2 Technical Capability: The overall technical requirements and proposed implementation for satisfying the technical requirements contained within the PWS:

a. The proposal shall address their understanding of the requirements and goals set forth in the scope of the work identified in the PWS, as well as a plan for accomplishing each requirement and goal.

b. The proposal shall address how schedules will be maintained for the technical solutions, requirements and goals identified in the PWS.

c. The Contractor shall include a staffing plan which explains how the contract will be staffed. The Contractor’s plan shall discuss the following:

1. The plan should contain the following information: position and location; tie-back to task(s) in PWS with an explanation on how this position will be filled is required; and how the prime Contractor will verify the experience and education of the key personnel to meets the minimum requirements specified in Section five of the PWS. The Contractor should also address which labor categories shall be filled by subcontractor or its own employees. The Contractor should also address plans in the event of prime and/or subcontractor withdrawals or addition of resources required, and staffing updates, throughout the period of performance.

2. The labor mix assigned to the functional program areas, that includes position name, level (i.e. Junior, Mid-level, or Senior employee), and Secret clearance level. A table may be best to summarize this information.

3. Furnish separate plans of operation in final draft format, as required in the PWS. Furnish an organization chart showing lines of authority, responsibility, and communication for management, supervisory, and technical personnel. Indicate what job classifications will be accomplishing the work items. Personnel management information such as hiring/firing authority, discipline, incentive plans, and retention.

d. The proposal shall provide copies of resume(s) for all of the key personnel identified in PWS Section 1.5.10 with a Letter of Commitment for all Key Personnel, whether employed by the prime Contractor or by a subcontractor. Resumes shall also affirmatively demonstrate the full time ability and security clearance for each person being proposed in the labor categories. Failure to submit a Letter of Commitment for those Key

Personnel may result in an unacceptable rating for Technical Capability. Letters of Commitment should be included as separate documents within the Key Personnel section and will not be included in the page count.

L.2.2.3 Factor – Price

The Contractor shall provide prices for each item listed in Section B. The breakdown of prices shall provide sufficient detail for the Government to identify the work effort and resources proposed to accomplish the work for each CLIN and evaluate the proposed prices for price reasonableness. Prices shall be valid for all contract years. The price proposal shall include: Name, telephone number, and e-mail address of the point of contact at the Contractor’s facility, and name, title and signature of authorized representative. Failure to provide detailed pricing may be grounds for proposal nonconformance. While price will not be scored, it will be evaluated for price reasonableness. Additionally, a determination will be made if there is any significant performance risk to the Government because of unrealistically low or high prices. If price information is included in any volume other than the Price Volume, that volume may not be evaluated.
