About this file
This is a Past Performance Questionnaire template (Attachment 7a) for solicitation HQ003425R0009 - DSCA International Loan Modeling & Budget Management. The questionnaire is divided into two sections: Section A to be completed by the offeror and Section B to be completed by the reference evaluator.
Section A requires offerors to provide company information, contract details including award date, period of performance, contract type, dollar values, and role on the project. Section B asks reference evaluators to rate the contractor's performance across six key areas using a five-tier rating scale (Exceptional to Unsatisfactory): Quality of Product/Service, Schedule, Cost Control, Business Relations, Management of Key Personnel, and Utilization of Small Business. Each rating area includes space for detailed comments, and the form concludes with a section for additional comments and signature.
