About this file
This document is a Health Start Program Solicitation Clarification Request from Unlimited Potential, responding to specific queries from a state or local health agency regarding their method of approach for a home visiting program. The document involves multiple organizations including LINQAZ Community Resource Network, Hispanic Nurses Association, and partnerships with school districts (Roosevelt, Cartwright, Alhambra, Murphy, and Tonopah) and community organizations like TigerMountain Foundation, Father Matter, Mentor Kids, and others. Unlimited Potential is located at Brooks Academy and collaborates with the Southwest College of Naturopathic Medicine & Health Sciences.
The proposed program focuses on providing comprehensive health and social services through community health workers, with a commitment to enrolling and supporting clients in a targeted service area. Key program elements include offering four or more hours of monthly consultation from a Registered Nurse and Licensed Social Worker, maintaining 24/7 nurse access for emergencies, and conducting team meetings through virtual, in-person, and telephone methods. Unlimited Potential aims to avoid service duplication by leveraging the LINQAZ network, which allows participating organizations to track client services, and by directly engaging with other community organizations to integrate and coordinate support. The organization emphasizes collaborative impact and empowering participants through comprehensive service coordination.

Health Start Program Solicitation BPM002372 – Clarification Request Unlimited Potential Method of Approach
6.2.1.2.5. Describe the resources available to enroll clients from the identified neighborhood/community selected as the targeted service area. Identify gaps in health care and social/behavioral health services that exist in the targeted service area. Briefly explain the relationships the Offeror has established with community resources; Need more information about what resources are available in the proposed targeted service area and what agencies and services are available to provide support regarding referrals.
Unlimited Potential is a member of LINQAZ Community Resource Network. This is an alliance of culturally competent community-based organizations who have come together to improve the health and well-being of individuals and communities by providing social services, clinical services, and linkages to educational and employment resources. The network encompasses 93 unique programs connecting individuals and families to services. In addition, Unlimited Potential, is located on Brooks Academy in which 10 different non-profit organizations work together, supporting community members and providing a diverse gamma of services. Among the organizations located at Brooks Academy is the Roosevelt Health Center providing general primary and preventive care. This is one of the clinics developed by Southwest College of Naturopathic Medicine & Health Sciences (SCNM) as community clinic to serve patients with the best of conventional medicine (e.g. prescription medicines, lab work), preventative and alternative medicine (e.g. acupuncture, clinical nutrition, supplements, homeopathy). Together we are providing the education and tools necessary for this community to create a brighter future.
Additional resources include, partnership with several school districts such as Roosevelt, Cartwright, Alhambra, Murphy and Tonopah. We have an average of 85 actively engaged community health workers working in the area. The community health workers work closely with other organizations such as TigerMountain Foundation, Father Matter, Mentor Kids, Tanner, Asian Pacific Coalition in Action and many others in order to improve social determinants of health and improve access to care. Unlimited Potential staff and Board of Directors belief on collaboration and partnership. It is through that collective impact that we can achieve a positive change.
6.2.1.2.7. Describe how the required four (4) or more hours of consultation from a Registered Nurse and a Licensed Social Worker or Licensed Counselor per month will be provided by whom and how Community Health Workers (CHW) will participate in the consultation meetings; Information wasn't provided regarding how the required four or more hours of consultation from a RN would be arranged. Need more information.
Unlimited Potential will be contracting with two of the nurses (one lead nurse and one backup) from the Hispanic Nurses Association to work with the CHWs and Licensed Clinical Social
Worker in this program. CHW will have 24/7 access to the registered nurse via telephone line for emergencies. Unlimited Potential will contract with two Licensed Clinical Social Workers (one lead and one backup) who are in good standing with the Arizona Board of Behavioral Health Examiners. Joint CHW, licensed clinical social worker and nurse meeting will occur once month for review of cases and updates. Team meetings will be conducted on a combination of methods such as virtual, in-person and telephone. In addition, CHW will meet separately with licensed social worker and registered nurse for more subject specific conversations. Home based for classes and consultations will be at Unlimited Potential; however, in order to facilitate travel for participant CHW will visit clients home and will do a virtual connection with the team from the participant’s house as needed with social worker and/or nurse.
6.2.1.2.8. If another similar home visiting program exists in the neighborhood/community selected as the targeted service area that serves the same population, document the efforts of the proposed Health Start program and the other programs in the community, and how all activities will be coordinated to avoid duplication of services and staff; Need more information about the other programs that exist in the neighborhood/ community and how it will be coordinated to avoid duplication of services and staff.
As a member of LINQAZ, participating organizations can see what other services clients are receiving. It opens the opportunity for collaboration, reduce duplication and increase scope of services. Unlimited Potential will be working directly with these organizations to enhance services. Best way of sharing information is to engage on regular dialogue with the organizations. As an initial assessment, participants will be asked to provide a list of services received, they will be asked if they will like us to help with the integration of services to facilitate the process and create a plan of sharing information. Services received will be updated with each trimester and/or as participant’s need changes. Unlimited Potential will work to empowered its participants by developing the tools and demonstrating how to be advocates for themselves and their families.
