About this file

The document is a Health Start Program Solicitation Clarification Request for Yuma County, Arizona, focusing on home visiting services. The Health Start program participates in the Yuma Regional Partnership Council (YRPC), which includes agencies such as Yuma Regional Medical Center, Chicanos Por La Causa, and Arizona Western College Child Development Center. The document addresses service coordination in Yuma County and La Paz County, with a particular emphasis on avoiding duplication of services and establishing communication with existing community-based programs.

The program's approach involves monthly collaborative meetings to share agency information, network, and coordinate client referrals. In Yuma County, the Health Start program receives client referrals and coordinates with existing agencies to ensure comprehensive service coverage. For La Paz County, which is described as a "pre-natal care desert," the program plans to establish communication with local organizations like WIC, Arizona Western College, and local clinics. A key best practice is directly asking clients about their involvement in other programs to prevent service overlap, with the goal of providing targeted maternal and child health support across both counties.

Health Start Program Solicitation BPM002372 – Clarification Request

Yuma County

Method of Approach

6.2.1.2.8. If another similar home visiting program exists in the neighborhood/community selected as the targeted service area that serves the same population, document the efforts of the proposed Health Start program and the other programs in the community, and how all activities will be coordinated to avoid duplication of services and staff;

Provide what other home visiting programs exist in Yuma County and La Paz and how you will coordinate with existing programs to not duplicate services.

Health Start of Yuma County, participates in the Yuma Regional Partnership Council (YRPC) monthly meetings. During these meetings several community home visiting agencies come together with a purpose to share agency specific information, updates, as well as provide an opportunity to network with one another.

Agencies at the YRPC table include Yuma Regional Medical Center, Chicanos Por La Causa, and the Arizona Western College Child Development Center, which all have an interest in positive Maternal Child Health outcomes. An advantage of attending the monthly meetings is receiving several client referrals from these agencies to participate in the Health Start program; by attending these meetings Health Start becomes knowledgeable of existing community programs, to not duplicate services. Furthermore, through this network, our program is able to coordinate and refer out for the services that are not included in our scope of work.

La Paz County will be a new service area for our program. La Paz is known to be a pre-natal care desert, we intend to establish a presence and open lines of communication with established community based programs such as WIC, Arizona Western College and the local clinic to be kept current of the pre-natal landscape.

One of Health Starts best practice is asking clients what other programs they may be involved with, to ensure we avoid duplication of services. This standard of procedure will be maintained when servicing both counties.
