6.1 Task 1: Preventive Maintenance for Kitchen Equipment. The contractor shall perform preventive maintenance services on kitchen equipment in the commons building of DISA HQ. A list of currently installed equipment has been provided at Attachment A. Preventive maintenance is defined as those procedures needed to keep the machine in peak operating condition including oiling, sharpening, adjusting, calibrating, etc. Maintenance will be performed in accordance with the equipment manufacture’s recommendations and will include: checking with the operating personnel to identify deficiencies, verification of proper operation of controls, thermostats and safety devices, condenser coil cleaning, part and filter replacement as needed up to $200.00 per item, and any other service needed to maintain proper operating condition. Per manufacturer specifications, most equipment should be checked and maintained on a quarterly basis; however minimum preventive maintenance intervals are shown on attachment A for each piece of equipment. This is a firm fixed price task.
The following deliverables are required under this task (reference section 11 for submission timelines):
· Preventive Maintenance management plan and schedule
· Monthly Preventive Maintenance report
6.2 Task 2: Kitchen Equipment Repair Service Calls (Non-Emergency).
Repair is defined as overhaul, reprocessing, or replacement of constituent parts or materials that have deteriorated by action of the elements or usage and have not been corrected through maintenance or replacement of the entire unit or system if beyond economic repair. Repairs shall replace failed components and restore the equipment to fully operational status. In the event equipment is in need of repair, the COR will determine the category of the repair (emergency, urgent, or routine, as defined below) and will contact the contractor.
Service Calls shall be responded to within a timely manner typically scheduled within the timeframes outlined below. This can involve contractor labor to resolve issues or include repairs or replacement to minor/ancillary components of the equipment in order to bring the piece back to full normal operation. Work shall be performed during normal operation times on Monday – Friday, no holidays or overtime is permitted. The following historical Service Call Data is provided for offeror’s information. The government has used the historical service call data to determine the dollar ranges of the service call levels under unscheduled maintenance and repair service calls:
Using the most recent data available (November 2015 through June 2016) the following service calls and repairs were made (These cost ranges include parts and labor.):
· Service Calls 12 per year. Note: A majority of these calls will be performed as preventive maintenance under this PWS.
· Minor Repairs ($501 to $2,500) 14 per year.
· Major Repairs ($2,501 to $5,000) 6 per year.
Note: No Emergency Calls were made during this period
Unscheduled Maintenance/Repair Service Calls will be made by the COR or designated government representative. Estimate shall be provided by the contractor on tasks to be performed, repairs, or installs relating to current equipment condition and/or recommendations to improve the condition of the system. The COR will verify price fair and reasonableness prior to the start of work which will then determine the level of the service call. A firm fixed price (which will include all labor and materials) will be awarded for all calls which fall under each of the specific level categories. The government has determined the amount to be awarded under each level and the contractor agrees to invoice only the awarded amount for any call that falls under each of the levels. For instance, for a call estimated by the contractor and concurred to by the COR to be $80.00 and a call estimated by the contractor and concurred to by the COR to be $500.00, each call would be billed at service call level I. The contract will be funded with a not to exceed amount for each service call level. The contractor is only authorized to invoice the exact number of calls authorized by the COR. It is the COR’s responsibility not to place calls and the contractor’s responsibility not to except calls that would exceed the current funded amount for each level. The service call levels are as follows:
| LEVEL | ESTIMATE | PAYMENT | ||
| I | $0-$500 | $ | ||
| II | $501- $2,500 | $ | ||
| III | $2501 -$5000 | $ |
The contractor shall document all unscheduled (non-emergency) repairs within the monthly repair report; this report shall outline the services performed, any findings relating to equipment condition and/or recommendations to improve the condition of the equipment, and a record of all service calls placed. This deliverable shall be provided to the Contracting Office Representative (COR) as outlined in the below chart, task 11.
Examples of Unscheduled Maintenance Services fitting the Service Call subcategory would include:

· Open air cooler not maintaining temperature, compressor needs replacement
· Condenser fans of cooler needs trouble shooting, Freon or repair.
Invoices for Service Calls shall include the date of service, the standard Service Call rate, and the vendor’s materials cost information. The contractor shall track accruing costs of the service calls on a monthly basis and provide the cost report to the COR and site technical point of contact each month. It will be necessary to report the costs within 5 calendar days after completion of the month which work occurred to ensure that the site does not exceed these predetermined limits.

The process will start with the contractor notification of defective parts to the COR and/or ACOR. Upon COR/ACOR review of the situation, a determination will be made on whether or not to move forward with the part correction. If the COR/ ACOR determines the part needs to be corrected, a modification package will be submitted to the contracting office. The KO will request a TR proposal from the contractor; the proposal shall include product costs and labor hours to complete such an installation; Upon negotiation of the final modification price, the Contracting Officer shall add these requirements to the current year contract by modification of the contract before the beginning of any work taking place.
