Past Performance Evaluation Survey Please return directly to: nswk2@socom.mil and Michelle.L.Riisma.civ@socom.mil with “PP Reference for M-SOFS RFP H92240-20-R-0001” in the subject line.
Dear Sir or Madam:
The mission of the Naval Special Warfare Command (NSWC) is to provide maritime special operations forces to conduct full-spectrum operations, unilaterally or with partners, to support national objectives. NSWC requires services to provide maritime subject matter expertise through logistics support services, equipment related services and knowledge based services in support of NSWC’s enterprise requirements for Maritime Special Operations Forces (M-SOF) missions. Services shall be provided in various locations throughout the United States of America and potential Outside the Continental United States of America (OCONUS). An offeror interested in proposing on this work has identified you as a Point of Contact (POC) on a past or present contract which the offeror deems relevant to this effort. We are, therefore, requesting your assistance in completing the attached Past Performance Assessment Questionnaire so that we may evaluate the offeror under the past performance factor.
Please provide your comments regarding the overall assessment of the offeror’s performance on the contract identified and any additional information that your organization deems relevant to our evaluation team. It is important that your information be as factual, accurate and complete as possible to preclude the need for follow-up by the evaluators.
Please complete and submit the Questionnaire as soon as possible via e-mail to the address above or contact Michelle Riisma at 619-243-4525 if you have any questions. Thank you very much in advance for your assistance.
