GENERAL DESCRIPTION OF REQUIREMENTS
This Blanket Purchase Agreement (BPA) is to provide survey services to determine the condition of asbestos-containing material (ACM) and lead based paint (LBP) containing materials by conducting testing for abatement of all hazardous material. Additionally, services are also needed to provide air abatement monitoring of a third party contractor who will perform the ACM and LBP removal.
General responsibilities include, but are not limited to, testing for abatement of all hazardous materials, identification and location of lead containing materials, conducting pre-abatement meetings with the general contractor, reviewing current ACM building reports, collecting pre-air samples to establish a clearance level, conducting visual inspections, providing daily air samples in and adjacent to the containment areas and providing daily air monitoring reports to both the GSA Project manager and GSA Field Office.
A sample statement of work is provided to further demonstrate typical project requirements and should be used as a guideline for determining which labor categories will be needed for service performance.
SAMPLE STATEMENT OF WORK:
SECTION 1 - ASBESTOS AND LEAD SURVEY SERVICES:
1. The Contractor shall furnish all necessary labor, materials, supplies, equipment, supervision, and laboratory facilities to provide comprehensive destructive asbestos and lead based paint sampling survey of federal office buildings in Orange and Los Angeles County. The main purpose of this Statement of Work (SOW) is to assess the condition of asbestos-containing (ACM) and lead based paint containing materials by conducting testing for abatement of all hazardous materials. Ensure that all components of all mechanical systems; all insulated piping systems; all floor, wall, and ceiling materials; and all other miscellaneous suspect materials have been ACM and LBP tested. This SOW will serve to support design efforts for estimating space alterations; to identify existing locations where asbestos work is needed; and to provide the information required to continually update the building Facilities Asbestos Action Plan (FAAP). The FAAP alone is not to be used to determine the presence and location of ACM for purposes of conducting demolition or renovation projects. It is mandatory that a comprehensive destructive survey be performed prior to conducting demolition or renovation operations.
Detailed instructions are contained in Part 5 which is titled WORK REQUIRED AND REPORT PREPARATION.
As a minimum, the following items need to be addressed in completing the SOW:
a. Review the building’s existing asbestos survey reports and floor plan drawings. This includes the existing
Facilities Asbestos Action Plan and any surveys and/or reports pertaining to repair and alteration or construction which may have affected the status of the asbestos. This is to be accomplished in the offices of the GSA Senior Property Manager (SPM) in charge of the buildings to be inspected. In some cases, visits to the buildings may be necessary to gain access to the building plans needed.
NOTE: The existing FAAP is known to contain erroneous information. This document should be used for reference only. All materials identified as None-detected “ND” are to be treated as “unknown” and are to be re-tested.
b. Prepare a listing of ACM to be re-inspected and re-assessed.
c. Contact the GSA Project Manager to arrange for reinspection of assigned buildings under the SP Manager’s jurisdiction. Discuss safety, health, and security consideration. Also arrange for government escorts or availability of keys and any other assistance needed from GSA.

3. Survey Requirement
Inspection shall be performed in accordance with the AHERA guidelines [40 CFR Part 763 Subpart E, Section
763.85 9(b)] except as modified in this Statement of Work. The AHERA guidelines allow an inspector to assume that certain materials contain asbestos. In general, this Statement of Work does not unless the Contractor is told by the SPM not to sample or the suspect material is not accessible. Use the AHERA 3-5-7 sampling protocol for sampling all surfacing material. Also, use the 3-5-7 rule for sampling drywall with joint compound and plaster. All other miscellaneous materials require at least 3 samples.
All suspect materials not previously identified as ACM or not previously sampled in accordance with current requirements must be sampled so that the number of samples meets this SOW. All sampling locations must be repaired
a. Reinspection activities accomplished under this SOW are intended to reassess the current condition of ACM and lead based paint on the project site and to obtain additional data as needed to develop a SOW for comprehensive abatement. ACM data on the building must be sufficient to support design efforts for space alterations on the project site and/or for the building mechanical staff to safely perform maintenance work on the building mechanical systems. The data must also identify existing locations where asbestos abatement work is needed.
b. Prepare written schedules for site visits to be made prior to each field trip. Provide copies of the schedules to the Contracting Officer and to the GSA Project Manager. Contact the GSA Project Manager to discuss the schedule and work out any problems. These schedules may require occasional revisions for some buildings due to temporary non-availability of GSA escort personnel or because of sensitive Government activities in the buildings to be inspected. The necessity for this shall be determined by the GSA SPM in charge of the buildings.
c. In addition to submittal of the schedules, personally contact the GSA Project Manager to coordinate the site visits. Verify availability of all assistance needed and contact the GSA Project Manager immediately if any delays or problems are encountered during the completion of the survey. Also, immediately contact the GSA Contracting Officer.
Note: Failure to properly coordinate site visits with the GSA Field Office and Project Manager may result in failure to gain access to some areas because of security requirements. This would require additional visits to complete the inspections at no additional cost to the Government.
d. Review building plans and other building information as needed to properly plan the reinspection. This is to be accomplished in cooperation with the GSA Project Manager.
4. Abbreviations and Terminology:
Common abbreviations normally used in correspondence may be used when appropriate. Terminology and the following abbreviations, specific to asbestos and asbestos survey reports, are also for use in the reinspection reports. c. Each mechanical room or open mechanical area is an inspection area where the report will group all building equipment and suspect materials identified on the field inspection DATA RECORD
SHEET.
d. All door enclosures on one particular floor can be considered an inspection area if all of the enclosures appear to be identical with regard to potential for containing ACBM.
e. Complete the appropriate DATA RECORD SHEET for each inspection area. Ensure that all components of all mechanical systems; all insulated piping systems; all floor, wall, and ceiling materials; and all other miscellaneous suspect materials have been noted. Some DATA RECORD SHEETS are included at the end of this scope of work. Duplicate these for use as needed.
NOTE
If the Contractor has an in-house form normally used that provides all information required on the DATA RECORD SHEETS provided in this scope of work, the in-house form may be used.
h. Bulk sample laboratory reports will be needed to complete all information required in the DATA
RECORD SHEETS.
7. Bulk Sample Summary: Summarize bulk sample results in a table format. Incorporate data from the old report that is still current with the laboratory results of new samples taken. Group samples from the same inspection area together. EXAMPLE: List all old samples from the Boiler Room with the new samples taken from the boiler room. Number the samples consecutively throughout the table starting with number one for the first sample listed. Include a column in the table that will differentiate between the old and the new samples. This may be done by simply recording “OLD” or “NEW” or the contractor may use this column to record special identification numbers used by the contractor for internal purposes. The contractor will not be held liable for the reliability or accuracy of sampling results from the old report.
Collect any additional bulk samples needed in compliance with Federal, state, and local regulations.
a. Any sampling requiring the use of respiratory protection by the inspector must be accomplished after normal working hours if the inspection area is occupied. Air handling equipment serving the area must be shut down while samples are taken.
b. Materials in unoccupied office areas or storage/mechanical areas not normally occupied may be storage/mechanical areas not normally occupied may be sampled during normal working hours when there is little likelihood of fiber release. Any such sampling will be out of the sight of any building occupants.
c. The number of samples to be taken for asbestos analysis shall be in accordance with 40 CFR Part
763.86 except that no assumptions may be made regarding the asbestos content of various suspect materials. All suspect materials not previously tested must be sampled and analyzed.
d. In each inspection area, a minimum of three samples shall be taken in each homogenous area contained in the inspection area of surfacing material or thermal system insulation (TSI). This is except in cases where only one or two samples were previously taken and analysis showed the material to be ACM. For surfacing material, the number of samples taken shall be increased, dependent on area of coverage, according to the guidelines in Part 763.86(a) as follows:
e. 1000 SF (93 SM) to 5000 SF (465 SM) - take 5 samples
f. Greater than 5000 SF (465 SM) - take 7 samples
g. When TSI involves piping, the straight runs of pipe shall be one homogeneous area and the elbows and fittings another.
h. Suspect TSI on each different piping system in the inspection area shall be sampled, even when the TSI on the different piping systems appears to be identical. However, total number of samples can be combined for separate piping systems if they appear identical as long as there is one

sample from each piping system. Provide pipe diameter and mechanical system designator from the table of abbreviations.
i. TSI does not have to be sampled when the accredited inspector has determined the material is fiberglass, foam glass, foam rubber, or some other common non-ACM insulation. However, the type non-ACM insulation must be recorded in the DETAILED SUMMARY section of the report.
The inspector, however, shall take care through visual observation to ensure that no suspect materials underlies or is otherwise obscured by the non-ACM insulation.
j. Take at least one sample from each location of patched TSI that is less than 6 LF (1.8 LM) or 6 SF (0.6 SM). Locations of patched TSI larger than these parameters shall be guided by the general rules for homogeneous areas.
k. For plaster samples, always state if the sample is common plaster or a special decorative, textured, or acoustical plaster. Also state whether the sample is from a wall, ceiling or somewhere else, such as an over spray of common plaster on a building column. A description only stating, “plaster” is unacceptable. A complete description would be “common ceiling plaster”.
l. Plaster on a courtroom ceiling and/or on the walls may be considered the same homogeneous area unless it is determined that there is a history of plaster work with dissimilar mixing batches.
m. Plaster from one floor to the next, whether a common plaster or a special plaster, shall be considered a separate homogeneous area in buildings constructed prior to 1978. This applies even when the plaster from one floor to the next appears to be identical. The number of samples to be taken however is modified to be three per homogeneous area per floor.
n. Report size, color, pattern, and/or other identifying characteristics when floor and ceiling tiles are sampled.
o. When floor tiles are sampled, the associated mastics shall also be sampled. This also applies to ceiling and wall tiles anchored in place with mastic.
p. Carpets shall not be taken up to sample floor tile beneath the carpet. Inquiries shall be made, however, to determine the extent of coverage of suspect floor tile beneath the carpets. This suspect floor tile will be shown on the CAD drawings and labeled as suspect but not assumed.
Add a footnote that suspect material must be sampled prior to any disturbance or renovation of the material.
q. Check cooling tower framework, louvered slats, and filler material and sample as necessary.
Sample roofing materials when this can be accomplished without compromising the watertight integrity of the roof. If roofing materials cannot be sampled, the materials will be shown on the CAD drawings and labeled as suspect but not assumed. Add a footnote that suspect material must be sampled prior to any disturbance or renovation of the material. Other suspect materials to be sampled include various mastics and gaskets associated with piping, ductwork, and other mechanical system components. Also sample transite materials used in various structural applications.
r. Samples shall be taken to the substrate and efforts made to preserve any layering of different materials. Report the contents of each layer including non-asbestos materials identified by the laboratory. When sample analytical results for layered systems (gypsum drywall with mud and joint compound) are provided for each separate layer and a composite result is provided, ensure that all other summary reports (for the same sample or homogeneous area) provide the same results for each layer, in addition to the providing the composite result.
s. Demolition to reach inaccessible chases and the inside of walls is not required. However, it shall be noted that they were not sampled and therefore must be evaluated before new renovation or demolition of unsampled areas.

t. Where possible, in occupied areas, sampling shall be in out of the way and out of sight locations to minimize the visibility of damage from the sampling.
u. All reasonable precautions shall be taken during sampling to avoid or minimize the release of fibers into the air. Under no circumstances shall sampling be accomplished under conditions where the activity can be observed by building occupants.
v. Accessible electrical insulation shall be inspected but not sampled. Suspect material found shall be reported.
w. Sampling shall not be done that would compromise structural integrity or otherwise result in significant adverse effects on the function the suspect material is intended to perform.
x. Record the sample identification number on the repair tape, other repair material, or on the piping or equipment surface by the sample location with an indelible marker or equivalent method.
y. Sample identification numbers need not be recorded at the locations where surfacing material or miscellaneous materials are sampled.
8. Summary of Homogeneous Areas: List homogeneous areas in the building as determined from information on the DATA RECORD SHEETS. Summarize the dimensions of each homogeneous area in square feet; linear feet; or in the case of pipefittings, total number of fittings. If room numbers differ from room identifications used by the surveyor, a footnote describing the difference is required.
a. Always PROMINENTLY label drawings to indicate if the drawing is only the basic floor plan or indicate the type of ACM information shown. EXAMPLE: “SECOND FLOOR VAT & TSI”.
b. Several drawings may be required for each floor, depending on the survey findings for a particular floor. Details are as follows:
(1) Basic floor plans.
• Show building columns.
• Show exterior walls and principal interior walls having slab-to-slab construction with heavier lines or some other means that will readily distinguish these from other walls and partitions.
• Show floor and room numbers. Room names may be used if there are no room numbers. In some cases, where room numbers are not shown at the doorway or entry way to a space, it may be necessary to check old drawings in the building mechanic’s office or in the GSA SP Manager’s office to find the room numbers.
• Show doorways and entryways.
• Show stairways, elevator shafts, and principal mechanical chases.
• Show the outlines of principal pieces of mechanical equipment in mechanical areas such as boilers, chillers, air handling units, and water tanks. Identify items shown including the equipment identification numbers (AHU #8, chiller #2, etc.) For complex mechanical areas, where this would badly clutter the drawing, provide a separate drawing or an insert on the basic drawing for the mechanical area.
• Provide two bar scales for distance and an indicator for the direction of North. One of the bar scales shall show linear measurement in feet and the other shall show linear measurement in meters.
• Show complete building identification, the contractor’s company identification, and the date the drawing was completed.

(2) Basic floor plans to which locations of TSI/ACM have been added. These plans shall also show locations where bulk material samples were taken. Also indicate when the samples tested positive (+) or negative (-) for asbestos content. The TSI/ACM floor plans, in additional to the information indicated above, shall show the following:
• Locations of vertical pipe risers. As near as possible, identify wall and chase locations for floors where the pipes are known or suspected to be concealed and inaccessible inside of the walls and chases. This is important for future O&M and renovation work in the building.
• Locations of horizontal runs of pipe with ACM insulation outside of the mechanical areas. Do not show pipe runs outside of mechanical areas if the pipes have only non-ACM insulation.
• The numerous individual pipes inside of the mechanical areas do not have to be shown, with the exception of the vertical risers. Ensure, however, that all piping in each mechanical area has been identified and that quantities are reported in the DETAILED SUMMARY section.
• Show air ducts and boiler exhaust stack, inside of mechanical areas if insulated with ACM. Outside of mechanical areas, only show air ducts and boiler stacks that are insulated with ACM.
(3) Basic floor plans to which coverage by VAT and/or ACM mastic has been added.
Indicate if only the mastic is ACM. These plans shall also show locations where the VAT and mastic samples were taken. Show coverage by suspect VAT that was not sampled because it is covered by carpet. Do not show coverage by floor tile and mastic confirmed to be non-ACM but do show where the samples were taken that confirmed these materials to be asbestos free.
(4) Basic floor plans to which locations and coverage by ACM textured/acoustical plaster;
common wall and ceiling plaster that are ACM; or other surfacing ACM have been added.
These plans shall also show locations where bulk material samples were taken. If no ACM plaster was found, only show sample locations. However, show even non ACM surfacing material on the drawings.
(5) Basic floor plans to which coverage by ACM suspended ceiling tile has been added.
These plans shall also show locations where the bulk samples were taken. Show sample locations but do not show coverage by non-ACM Ceiling tile.
(6) In the reports, keep the plans for the same floor grouped together in the sequence given above.
(7) In cases where information on more than one type of ACM can clearly be shown without cluttering the drawing, then only one drawing may be used. If no ACM was found on a floor provide just one drawing (in addition to the basic floor plan) with a note prominently stating, “NO ACM IDENTIFIED ON THIS FLOOR.”
(8) The footers showing building identification will not be shown since the building identification will be shown on the drawings. The page numbers, however, must be shown on each drawing and may be handwritten.
c. The small floor plan copies furnished by GSA shall not be copied for attempted use in the survey reports. The 8 ½ x 11 plans, when available, are provided as an aid during the building inspection and for reference when preparing the CAD drawings for the reports.
d. CAD drawings for the reports do not have to be completed with the precision and detail that would be expected in engineering drawings however all details and information must be legible with the un-aided eye. They should however, be accurate enough for use in making area estimates or for approximating quantities of piping or other items for which measurements would be used.

e. 8.5” x 11” drawings are preferred in the reports when this smaller size will not obscure details.
However, 11” x 17” drawings will be acceptable if this would be beneficial in showing details. If 11” x 17” drawings are used, the report must be three-hole punched on the side to allow the drawings to fold out from a standard folder or three ring binders.
15. Photographs. Take representative photographs for current and previous surveys where samples were taken of both ACM and non ACM materials from each homogeneous area and identify the location in the photograph as noted below. Clear, sharp photographs are required. Blurred or fuzzy photographs are unacceptable. The photographs may be in color or in black and white. When color film is used, the photocopies used in the final reports must be in color.
a. Photographs shall be of a size so that two photographs, with captions and footers, will fill one page.
These shall be photocopied for inclusion in the reports. The photocopies in the final reports must be comparable in quality and clarity to the original photographs.
b. Use stamped arrows, neatly printed arrows, or other means to show locations where samples were taken. Show the sample numbers. If all sampling locations are not clearly shown on the area photographs, take additional photographs, as needed, to show where samples were taken.
c. Use stamped arrows, neatly printed arrows, or other means to show locations of damaged ACM. In all cases of damage, take additional closer photographs to show details of the damage.
d. The photos shall be appropriately captioned to explain what the photograph is intended to show and each photograph numbered to facilitate cross referencing in other parts of the inspection report.
e. The footers showing building identification and report page numbers shall be shown on each page of the photograph section.
16. Management Plans:
a. Reinspection and Reassessment Plans.
(1) Summarize locations that will need periodic reinspection and list ACM items in each area to be inspected and reassessed.
(2) Provide an appropriate form, or forms, to be filled in by the inspector, which will serve as a record of the reinspection.

10. Communications with Tenants: The Occupational Safety and Health Administration's (OSHA) regulations 29 CFR 1910.1001 and 29 CFR 1926.1101 contain mandatory sections requiring facility owners and employers communicate hazards to employees/tenants. In GSA buildings the Facility Asbestos Control Manager, usually the SPM, is in charge of tenant communications/notifications. The heads of tenant agencies must be notified of the presence, location and quantity of friable and non-friable asbestos and the implementation of a special operations and maintenance program in the building. In addition, each tenant agency with authority to make alterations to the space it occupies must be notified of the presence, location and quantity of asbestos present and the applicable requirements. The FAAP should contain a statement that two weeks prior to the start of an abatement or alteration project that will involve asbestos that all affected agencies will be informed. OSHA requires that within 10 days of completing work covered by the standard, that the building owner and the employer of employees in adjacent areas be notified of the quantity and current location of ACM/PACM remaining in the area and any final monitoring results. This section should also contain the requirement that Warning Labels shall be attached immediately adjacent to any friable or non-friable ACM located in routine maintenance areas. All labels shall be permanently displayed in readily visible locations and shall remain posted until the ACM is removed. Under asbestos notifications to building occupants make the following statement "Re-notify tenant agencies if findings in the survey report that accompanies this O&M Plan are significantly different from asbestos information previously furnished."

12. Waste Disposal Procedures and Tracking of Hazardous Waste Manifests: All waste material containing asbestos must be properly containerized, labeled, stored and disposed of in accordance with Federal, State and local regulations at approved disposal facilities. Records, transportation manifests and other related documents are incorporated as part of this action plan. Asbestos containing waste material shall be packaged and disposed of according to the regulation. The SPM must retain copies of the Hazardous Waste Manifest of each shipment. Pertinent regulations include, but are not limited, to the following.
a. OSHA Asbestos Standard for General Industry 29 1926.1101
b. Department of Transportation (DOT) 49 CFR Parts 171 & 172 DOT regulates the transport of hazardous materials on public thoroughfares. Friable asbestos is considered a hazardous material with a reportable quantity of 1 pound. DOT requires proper packaging, labeling and transporting of all hazardous materials.
c. EPA regulations (40 CFR) National Emission Standards for Hazardous Air Pollutants (NESHAP) 40 DFR Section 61, Subpart M. governs asbestos air emissions during demolition and renovation projects.
