Collection of Task Order Confirmation Form Human Capital and Training Solutions (HCaTS)
Solicitation: GS02Q15CRR0001 Amendment Number: 01 Attachment J.9
The Government may contact you to verify or clarify the information provided on this form.
General: This form is to be sent to the Contracting Officer (or commercial equivalent Authorized Official for a Non-Federal government project) of a collection of task orders to request confirmation the collection of task orders identified herein in fact meets the requirements set forth in the Human Capital and Training Solutions (HCaTS) RFP Section L.5.2. The Government will use this information solely for source selection purposes under the HCaTS RFP.
Contracting Officers: You are receiving this form because the Offeror cited in Section I claims the identified collection of tasks orders in Section II meets the definition of a collection of task orders as prescribed in Section III. The HCaTS team is asking that you, as the duly warranted Contracting Officer (or commercial equivalent Authorized Official for a Non-Federal government project) for the collection of task orders cited in Section II-A, confirm the collection meets the requirements set forth herein. By completing this form, you are not required to make changes to any existing documents or systems related to the contract/task order. SECTION III. CERTIFYING OFFICIAL
I, as a duly warranted Contracting Officer (or commercial equivalent Authorized Official for a Non-Federal government project) of the task orders cited in Section II-A, certify that the above listed actions constitute a collection of task orders as follows:
The collection of task orders was placed under a master Single Award or Multiple Award, Indefinite Delivery task order contract or master Single Award or Multiple Award Blanket Purchase Agreement; or, any other master contract vehicle (e.g., Project Number/Title) that the task orders were directly awarded under if the following applies:
(1) The “collection of task orders” shall not exceed six task orders, and
(2) None of the task orders in the collection shall also be used as a stand-alone Relevant Experience Project, and
(3) Each successor task order was a logical follow-on task order to the predecessor task order, with no more than a 30 calendar day gap between the award date of the successor task order and the end date of the predecessor task order, and
(4) Each task order was awarded for the same customer, and
(5) Services provided under each task order had been performed within the past five years prior to the solicitation closing date (or be ongoing), and
(6) Each task order’s scope was inclusive of at least one KSA (Customized Training and Development Services, Customized Human Capital Strategy Services, or Customized Organizational Performance Improvement) and some component of the KSA-related service and/or product customized.
